Cette étude porte sur les temps de passage aux urgences adultes médicales et traumatologiques du CHU de Nantes. Elle est réalisée sur les 57879 passages enregistrés au cours de l’année 2007. Le nombre moyen de patients s’étant présentés aux urgences de Nantes est de 158 patients par jour. Il ressort de cette étude que le temps de passage médian aux urgences adultes est de 272 minutes et le délai médian avant la première évaluation médicale de 68 minutes. Nous avons pu individualiser plusieurs facteurs participant à l’allongement du temps total passé aux urgences : l’affluence des patients, maximale entre 10h et 22h, la gravité du motif de recours et l’âge. Différents axes peuvent être explorés pour réduire les temps de passage, par exemple : une adaptation de la présence des personnels médicaux et paramédicaux affectés, grâce à l'analyse des flux horaires et journaliers des patients ou la participation d'un médecin à l’accueil des urgences.

Objectif : analyse des critères de décision de LATA aux urgences d'un CHU et devenir des patients concernés. Matériel et Méthode : étude rétrospective monocentrique au CHU de Nantes, de l'ensemble des formulaires de LATA remplis aux urgences du 30 juin 2016 au 1er juillet 2017. Résultats : près d'un tiers des LATA du CHU ont été décidées au sein du service des urgences. Les patients étaient âgés (en moyenne 80,4 ans), majoritairement des femmes, en provenance de leur domicile, partiellement dépendantes, et porteuses d'un cancer, d'une démence et/ou d'une insuffisance cardiaque. Les défaillances d'organe les plus fréquemment présentées étaient respiratoire, neurologique et hémodynamique. Le principal motif de décision de LATA invoqué était la dégradation prévisible de l'autonomie future. L'expression de la volonté du patient n'était recherchée que dans 28% des cas. La collégialité de la décision n'était pas respectée dans 37% des cas. Les médecins consultants les plus sollicités étaient urgentiste (46%) ou réanimateur (20%). Les patients étaient hospitalisés à l'UHCD dans 59% des cas. 59% ont reçu des soins palliatifs. 61% des patients sont décédés lors de leur séjour hospitalier, dont 6,5% sont décédés dans le service des urgences. Les délais entre admission et LATA, LATA et décès, et admission et décès étaient respectivement de 4,6h (± 5,53h), 33,6h (± 1506h) et 41,2 (± 1506h). Conclusion : la LATA n'est pas synonyme de décès précoce. Des améliorations sont à attendre dans le respect du cadre réglementaire, notamment en terme de collégialité et de recherche de la volonté du patient, de la traçabilité, et de la formation des médecins à la loi, en éthique et aux soins palliatifs.

la surcharge aux urgences est un problème actuel. Le circuit debout permet de prendre en charge des patients légers. Matériel et méthode : L'étude était prospective, mono-centrique, observationnelle et descriptive. Les patients du CD admis du 31 aout au 7 septembre, hormis les IPM ont été inclus. Le critère de jugement principal était l'épidémiologie de la population. Les critères de jugement secondaire étaient l'étude des temps de passage et la répartition sur le nycthémère. Résultats : 361 patients ont été inclus, 54 exclus par manque de données. 182 hommes âgés de 35 ans en moyenne, et 125 femmes âgées de 34 ans en moyenne. 64,8 % sont venus le jour, 58% pendant la permanence des soins. 78,5% des patients venaient sur leur propre initiative. Dans 56% le motif était traumatologique. 60% des dossiers ont été séniorisés. 79% des patients ont eu au moins un examen complémentaire. 80% des patients ont été classés CCMU 2 et 89,3% GEMSA 2. Le temps de passage moyen était de 152 minutes : 197 minutes pour la médecine et 119 minutes pour la traumatologie. La prise en charge des patients était plus rapide pendant la PDS. Le taux d'hospitalisation était de 4,2%. Conclusion : Au vu de ces résultats, nous proposons sept axes d'amélioration : la mise en place de critères d'admission, la prise en charge des patients par la TAC lors de la fermeture, l'adaptation des horaires des internes, une sensibilisation à la gestion des examens complémentaires l'ajout d'une compétence traumatologique à la permanence des soins et la modification du poste du MAO. Ces éléments pourront être évalués dans un prochain travail.

Central banks came out of the Great Recession with increased power and responsibilities. Indeed, central banks are often now seen as 'the only game in town', and a place to put innumerable problems vastly exceeding their traditional remit. These new powers do not fit well, however, with the independence of central banks, remote from the democratic control of government. Central Banking in Turbulent Times examines fundamental questions about the central banking system, asking whether the model of an independent central bank devoted to price stability is the final resting point of a complex development that started centuries ago.
