Contexte : la santé sexuelle est considérée comme intégrante de la santé humaine et une problématique de santé publique. À ce titre elle relève de la fonction du médecin généraliste. Mais son intégration dans une pratique de médecine générale n'est pas évidente. Objectifs : décrire le vécu de généraliste concernant l'abord de la sexualité dans leur pratique, en identifier les freins et définir des points clefs de formation. Méthode : étude qualitative par des entretiens semi-dirigés auprès de 20 médecins généralistes picards. Analyse selon la méthode de la théorie ancrée. Résultats : la pratique sexologique était très hétérogène. Elle était liée à la singularité de chaque dyade relationnelle médecin-patient selon leur âge, sexe, relation et conception de la place de médecin généraliste. L'initiateur du thème sexologique était le médecin, le patient ou un tiers. La demande et les réponses touchent aux sexopathies mais aussi au comment ça marche, que faire, est-ce normal et à des besoins d'être reconnu, déculpabilisé, libéré. Cela mobilisait des affects chez le médecin notamment de gêne, besoin de se protéger, questionnement personnel, et pouvait modifier la relation. Le généraliste développait un professionnalisme avec l'expérience. Discussion : la pratique sexologique du généraliste se différencie de celle du sexologue au niveau de l'origine et de la nature des demandes et des réponses. Elle est intégrée à une prise en charge qui dépasse la sexualité et dans le cadre d'une relation et d'une position du médecin très particulières. Conclusion : la spécificité de la pratique sexologique du médecin généraliste justifie chez celui-ci un savoir sur les sexopathies mais tout autant un travail de réflexion éthique, philosophique, sociologique, et sur la relation médecin - patient.
