L’atteinte médullaire traumatique est une pathologie grave et invalidante qui nécessite le plus souvent une sanction chirurgicale. Cette étude tente de préciser les liens entre niveau lésionnel, complications respiratoires, chirurgie dans les 24 heures et récupération motrice. Nous avons étudié 417 atteintes médullaires séparés en deux groupes différents : les atteintes complètes (n=288, 69%) et incomplètes (n=129, 31%). Ces patients ont été pris en charge dans 20 centres hospitaliers de 2001 à 2003 et suivis pendant 1 an. Une variation positive du score ASIA moteur de 10 points en 1 an a été considérée comme une récupération motrice minimale utile. Les comparaisons ont été faites entre les 2 groupes mais aussi à l’intérieur de chaque groupe suivant le niveau de la lésion. L’influence des épisodes d’hypotension, du délai opératoire et des complications respiratoires a été testée sur la récupération motrice à 1 an. Résultats : La récupération fonctionnelle est très différente entre les deux groupes en quantité (24% pour le groupe complet et 85% pour le groupe incomplet) comme en qualité (gain de 6±12 points versus 37±23 points). Quarante huit pour cent de l’ensemble des patients est opéré dans les 8 heures et 76 % dans les 24 heures. Aucun délai chirurgical ne semble favoriser la récupération importante des patients à l’atteinte incomplète. Le risque (non retrouvé) de compléter une atteinte partielle entraîne une attitude interventionniste rapide. Par contre, la prise en charge chirurgicale avant 24 heures est statistiquement associée à une meilleure récupération motrice à un an pour les atteintes complètes (p=0,04), sans augmenter le nombre de complications non neurologiques. It is with great pleasure and ,much interest that I accepted to write the foreword to this book by Paul Doury, Yves Dirheimer, and Serge Pattin on the subject of "algodystrophy." First, because I know the extent of their personal experience, from which they have selected the best for this book. Second, because it seemed to me that their detailed analysis of the numerous works on the subject, works which have been published all over the world and which provide diverse physiopathologic interpretations, would provide a comprehensive study meeting a real need. Algodystrophy, to adopt the term used by the authors, merits rheumatolog ists' careful attention. It is indeed a frequent condition and, as is now well known, occurs in the most varied etiologic circumstances; it is not solely posttraumatic, a notion on which diagnosis had long been based. This variable etiology suggests the complexity of algodystrophy's pathogenic mechanism.
