The ? eld of critical care medicine is in the midst of a dramatic change. Technological and s- enti? c advances during the last decade have resulted in a fundamental change in the way we view disease processes, such as sepsis, shock, acute lung injury, and traumatic brain injury. Pediatric intensivists have been both witness to and active participants in bringing about these changes. As the understanding of the pathogenesis of these diseases reaches the cellular and molecular levels, the gap between critical care medicine and molecular biology will disappear. It is imperative that all physicians caring for critically ill children in this new era have a th- ough understanding of the applicability of molecular biology to the care of these patients at the bedside in order to keep up with the rapidly evolving ? eld of critical care medicine. To the same extent, the practice of critical care medicine is in the midst of fundamental change. The increased provision of facilities for neonatal and paediatric care in the last 25 years has been accompanied only in part by appropriate developments in pathology. Specialist pathol ogists are many fewer than paediatric departments, and details of the advances in knowledge of the pathogenesis of diseases in childhood and of ways of investigating them are not uniformly available. In many institutions an individual with a special interest rather than a special training will be responsible for paediatric pathology and it is to this group of histopathologists that this text is addressed. For this reason it is not written as a comprehensive text and is not intended for use as a reference volume. Areas which may produce particular difficulties for the individual with little specialist knowledge of the very young (e. g. the lung) are dealt with in more detail and, in general, entities in which the histopathology does not differ greatly from that of the adult disease are considered briefly. A brief account of developmental processes is included where prenatal considerations are helpful in understanding a particular entity. Al was the senior editor of this textbook, which he founded with Harry Feldman in 1982. Al was a clinician, epidemiologist, educator, catalyst for biomedical research, historian, author, speaker, seeker of the truth, sincere friend of students, sports enthusiast, traveler, and truly a man of all seasons. He was a devoted husband to Brigette Klug Evans, father of three children, and grandfather of four. Al was born in Buffalo, New York, on August 21,1917, to Ellen Spring and John H. Evans, M.D., one ofthe United States's first anesthesiologists and an early researcher in the field of oxygen therapy. He received his undergraduate training at the University of Michigan; was awarded an M.D. degree in 1943 from the University of Buffalo; interned in Pittsburgh, Pennsylvania; and performed his medical residency at the Goldwater Hospital in New York City.
