California agriculture relies heavily on hired farm workers, up to 1 million/year. Majority of these workers are Latino, which is the largest and fastest growing minority group in America. Their health status has not been well measured. Historically, health knowledge about the farm workers is incomplete. Injury is one of the adverse health outcomes due to agricultural work exposures. The purpose of this study is to carry out a longitudinal cohort study of hired farm worker families in California Central Valley; determine the prevalence and distribution of injury; examine common work positions may increase the risk for chronic musculoskeletal pain; assess psychological factors may be associated with an increased risk of injury; and explore the possible causal link between physical activity and injury.A total of 759 Latino farm workers living in Mendota California were included from the MICASA study and completed the baseline interviewer-administered questionnaire. Chronic musculoskeletal pain is prevalent among farm workers and is associated with common work positions. A higher prevalence of last 12-month injury was observed among participants older than 40 years, current smokers, those who had worked in agriculture for more than 11 years, and who had resided in the U.S. more than 21 years. Next, we assess the association between psychological factors and injury among the same sample and we found that depression and nervios were significantly associated with injury. Finally, a total of 640 Latino farm workers who completed the both baseline and follow-up questionnaire were included in the longitudinal data analysis. A generalized estimating equations (GEE) approach was used to analyze longitudinal data of binary outcome (injury) to account for correlation between the repeated observations within participants. And we found that physical activity was not associated with injury among Latino farm workers.The results of this study show that chronic musculoskeletal pain is common among farm workers and is influenced by work positions. The findings suggest that preventive interventions for agricultural tasks associated with increased pain will likely need to focus on engineering changes and limitations on time spent at certain tasks. We also found that psychosocial variables were strongly associated with injury. The findings may be useful for guiding prevention and management of injury providing psychosocial treatment may reduce injury risk.
