Introduction : Le suivi de pathologie chronique est un motif fréquent de consultation en médecine générale, que les médecins remplaçants sont amenés à faire en l’absence du médecin traitant. Etant un mode d’exercice important en France actuellement, nous avons souhaité connaître le ressenti de ces médecins remplaçants lors des consultations de renouvellement d’ordonnance. Méthode : Une enquête qualitative par entretiens semi-dirigés, a été réalisée auprès de 12 médecins généralistes remplaçants, inscrits sur les listes de l’Ordre des médecins, d’octobre 2012 à avril 2013, enregistrés et transcrits intégralement puis analysés manuellement. Résultats : Les médecins généralistes remplaçants organisaient la consultation de renouvellement d’ordonnance de la même manière. Ils souhaitaient presque tous simplifier les ordonnances. Plusieurs facteurs facilitant les modifications intervenaient, tenue des dossiers, formation initiale, les dernières lectures, la relation avec le patient. Des freins sont apparus, comme le manque de confiance en soi, le souci de confraternité. Certaines pathologies, notamment psychiatriques semblaient soulever plus de difficultés. Conclusion : La consultation de renouvellement d’ordonnance peut se révéler complexe, et faire naître un conflit intérieur chez certains médecins remplaçants, limitant parfois les modifications de traitement.
