La ventilation non invasive, technique d'assistance ventilatoire sans dispositif endo-trachéal, a connu plusieurs phases de développement : le poumon d'acier, les systèmes de ventilation en presssion positive utilisés pendant la nuit puis dans le cadre de détresses respiratoires avec début de la CPAP à la fin des années 1980. Les causes de détresses respiratoires aiguës sont diverses : les décompensations de broncho-pneumopathies chroniques obstructives et l'oedème aigu pulmonaire cardiogénique sont ciblés dans le cadre de la prise en charge en urgence. Il existe plusieurs modes ventilatoires : les ventilations assistées, contrôlées en volume ou en pression, proportionnelle et spontanée avec pression expiratoire positive (CPAP), plus ou moins aide inspiratoire (BIPAP). La VNI peut être délivrée par des ventilateurs de réanimation ou portables, ou des systèmes indépendants type valve de Boussignac. L'interface utilisée en premier lieu est le masque facial ; il doit être le moins traumatisant et le plus étanche possible er représenter un faible espace mort. Les complications liées à l'intubation sont évitées et le travail ventilatoire est diminué. La PEP améliore les échanges gazeux, par recrutement alvéolaire, et les performances ventriculaires gauches. L'aide inspiratoire augmente le volume courant et diminue la fréquence respiratoire. Les complications majeures sont dues aux fuites : désynchronisation entre patient et machine et hypoventilation. Il peut exister aussi des lésions oculaires et cutanées qu'il faut savoir prévenir. Les indications principales en médecine pré-hospitalière ou d'urgence sont les décompensations aiguës de BPCO, l'OAPc, comme l'ont prouvé plusieurs études tant au niveau efficacité que faisabilité. L'équipe soignante doit être formée à la technique et pouvoir faire face à un échec avec recours à l'intubation. Ceci implique de bonnes connaissances anatomo-physiopathologiques et cliniques et la nécessité d'un protocole de mise en oeuvre.

La ventilation non invasive ou VNI est une technique de ventilation dont l'efficacité n'est plus à démontrer en réanimation. Depuis la création des unités de surveillance continue (USC) au début du XXIème siècle, la VNI tend à s'imposer. L'objectif principal de cette étude est de définir le taux de succès de la VNI. Les objectifs secondaires sont d'évaluer les indications de la VNI au sein de l'USC des Hôpitaux Drôme Nord et d'identifier des facteurs de risque d'échec de VNI. Nous avons mené une étude rétrospective, observationnelle, monocentrique sur 42 mois. Mille huit cent deux patients ont été inclus et analysés. Sur 757 ayant présenté une défaillance respiratoire aiguë, 33% ont bénéficié de la VNI. Nous avons constaté une augmentation croissante dans l'utilisation de la VNI passant de 11% à 18% entre 2010 et 2013. Le taux de succès est de 67% en ne prenant en compte que les patients vivants, non intubés, non transférés en réanimation. Le taux d'intubation secondaire est de 22%. Nous avons pu identifier des facteurs de risque d'échec de VNI (le score IGS II, la défaillance cardio-vasculaire, la pneumopathie infectieuse et le sepsis d'origine extra-pulmonaire) et des facteurs de succès (les antécédents de BPCO/IRC, la décompensation aiguë spastique de BPCO, le traumatisme thoracique). Notre population se rapproche d'une population de réanimation, 57% des patients présentant au moins une défaillance d'organe. Nous avons pu démontrer une bonne utilisation de la VNI en surveillance continue chez une population relativement grave. Nous souhaitons dans l'avenir améliorer les procédures paramédicales afin d'optimiser la prise en charge des patients.

The Governor's Dilemma develops a general theory of indirect governance based on the tradeoff between governor control and intermediary competence; the empirical chapters apply that theory to a diverse range of cases encompassing both international relations and comparative politics. The theoretical framework paper starts from the observation that virtually all governance is indirect, carried out through intermediaries. But governors in indirect governance relationships face a dilemma: competent intermediaries gain power from the competencies they contribute, making them difficult to control, while efforts to control intermediary behavor limit important intermediary competencies, including expertise, credibility, and legitimacy. Thus, governors can obtain either high intermediary competence or strong control, but not both. This competence-control tradeoff is a common condition of indirect governance, whether governors are domestic or international, public or private, democratic or authoritarian; and whether governance addresses economic, security, or social issues. The empirical chapters analyze the operation and implications of the governor's dilemma in cases involving the governance of violence (e.g., secret police, support for foreign rebel groups, private security companies), the governance of markets (e.g., the Euro crisis, capital markets, EU regulation, the G20), and cross-cutting governance issues (colonial empires, "Trump's Dilemma"). Competence-control theory helps explain many features of governance that other theories cannot: why indirect governance is not limited to principal-agent delegation, but takes multiple forms; why governors create seemingly counter-productive intermediary relationships; and why indirect governance is frequently unstable over time.
