Le cancer de l'enfant est une réalité pour environ deux mille enfants chaque année. Du fait d'une demande des familles et de l'évolution du système de santé le domicile apparaît actuellement comme une vraie alternative à l'hospitalisation. Je me suis donc intéressée, dans un premier temps, à déterminer la place des soins à domicile dans la prise en charge d'hémato-oncopédiatrie en m'appuyant sur l'expérience vécue par les enfants et leurs parents. Dans un second temps j'ai essayé de clarifier le rôle du médecin généraliste dans ces prises en charge spécialisées ambulatoires. J'ai réalisé une étude qualitative par entretiens semi dirigés auprès d'enfants atteints de cancer ou d'hémopathie et de leurs parents. Sept enfants et dix-huit parents ont été interviewés le plus souvent à leur domicile entre janvier et mai 2012. Dix-huit entretiens ont été entièrement encodés. Le domicile comme lieu alternatif à l'hôpital est plébiscité par les enfants et les parents. Il permet de préserver un confort matériel et affectif pour l’enfant qui conserve une vie sociale au plus proche de celle qu'il connaissait. Cependant il ne doit pas exister de perte de chance pour l'enfant et l'étayage à domicile doit être étoffé. Les parents sont projetés en position de soignants et ils sont responsables de la surveillance de l'état de santé de leur enfant. Cette responsabilité est très insécurisante pour eux d'autant plus qu'ils s'interrogent souvent sur les compétences techniques du personnel para médical ambulatoire vis-à-vis de ces soins spécialisés. Un sentiment de solitude, voire d'abandon peut naître malgré le lien téléphonique permanent possible avec l'IHOP. Le médecin généraliste peu impliqué jusqu'à présent, pourrait apporter ses compétences spécifiques relationnelles, de soutien et de coordination dans cette prise en charge ambulatoire. Il connaît les familles, il peut se déplacer rapidement à domicile et assurer le suivi. Les soins à domicile ont leur place dans la prise en charge hémato-oncopédiatrique. Il est cependant nécessaire d'entourer et de soutenir les parents tout au long de cette prise en charge angoissante et épuisante pour eux. Il faudrait également réfléchir à une place pour le médecin généraliste tenant compte du domaine de ses compétences.
