One of the major causes of medication errors in prescribing is the use of potentially dangerous abbreviations.4 An abbreviation used alone, or combined with other words or numerals, may be misunderstood by the person interpreting the medication order or prescription.5 Little is known about the frequency and potential consequences of errors related to misinterpretation of medication information. [...] Depending on the size (based on the number of beds) and type of the hospital (which influences which types of medication orders are written), it may take several months to collect the minimum number of medication orders/prescriptions. [...] Multiple medications listed on one medication Enter each medication as a separate record in the order database (i.e., for one medication order there may be multiple records in the database) "As per protocol" medication orders If the protocol is faxed with the "as per protocol" order, then enter all available data on the protocol in the database. [...] In a previous study, it has been shown that sending reminder notices on medication orders using prohibited abbreviations to the healthcare providers reduced the number of medication orders with prohibited term.9 In fact, in that study the physicians received the most notices over the nurses and the pharmacists received none. [...] The database for this pilot study was developed in Microsoft Access and examples of the data collection forms are provided in Appendix A. For each medication order, a pharmacy technician or a pharmacy student should enter the following into the database: dose units, route of administrations and dose frequencies; whether the dose units, route of administrations and dose frequencies were handwritten.

This User’s Guide is intended to support the design, implementation, analysis, interpretation, and quality evaluation of registries created to increase understanding of patient outcomes. For the purposes of this guide, a patient registry is an organized system that uses observational study methods to collect uniform data (clinical and other) to evaluate specified outcomes for a population defined by a particular disease, condition, or exposure, and that serves one or more predetermined scientific, clinical, or policy purposes. A registry database is a file (or files) derived from the registry. Although registries can serve many purposes, this guide focuses on registries created for one or more of the following purposes: to describe the natural history of disease, to determine clinical effectiveness or cost-effectiveness of health care products and services, to measure or monitor safety and harm, and/or to measure quality of care. Registries are classified according to how their populations are defined. For example, product registries include patients who have been exposed to biopharmaceutical products or medical devices. Health services registries consist of patients who have had a common procedure, clinical encounter, or hospitalization. Disease or condition registries are defined by patients having the same diagnosis, such as cystic fibrosis or heart failure. The User’s Guide was created by researchers affiliated with AHRQ’s Effective Health Care Program, particularly those who participated in AHRQ’s DEcIDE (Developing Evidence to Inform Decisions About Effectiveness) program. Chapters were subject to multiple internal and external independent reviews.

The authors of this book set out a system of safety strategies and interventions for managing patient safety on a day-to-day basis and improving safety over the long term. These strategies are applicable at all levels of the healthcare system from the frontline to the regulation and governance of the system. There have been many advances in patient safety, but we now need a new and broader vision that encompasses care throughout the patient’s journey. The authors argue that we need to see safety through the patient’s eyes, to consider how safety is managed in different contexts and to develop a wider strategic and practical vision in which patient safety is recast as the management of risk over time. Most safety improvement strategies aim to improve reliability and move closer toward optimal care. However, healthcare will always be under pressure and we also require ways of managing safety when conditions are difficult. We need to make more use of strategies concerned with detecting, controlling, managing and responding to risk. Strategies for managing safety in highly standardised and controlled environments are necessarily different from those in which clinicians constantly have to adapt and respond to changing circumstances. This work is supported by the Health Foundation. The Health Foundation is an independent charity committed to bringing about better health and health care for people in the UK. The charity’s aim is a healthier population in the UK, supported by high quality health care that can be equitably accessed.
