Contexte et objectifs : Le Performance Status au diagnostic est associé à la survie globale chez les patients atteints d'un cancer colorectal métastatique, mais ne conserve pas sa valeur pronostique une fois le Fonctionnement Physique selon le QLQ-C30 pris en compte. Nous avons cherché à déterminer la valeur pronostique d'une détérioration définitive du Performance Status et du Fonctionnement Physique avant la première évaluation tumorale. Méthodes : Nous avons réalisé une analyse post hoc sur la cohorte de l'étude prospective observationnelle multicentrique française COBEQOL. Les patients analysés devaient avoir été traités avec un protocole de chimiothérapie comportant Oxaliplatine, lrinotecan, ou les deux, en adjonction au Bevacizumab. Le Performance Status et le Fonctionnement Physique devaient être disponibles à l'inclusion. Trois temps d'évaluation ont été pris en compte : l'inclusion, T1, et T2, respectivement définis comme le temps médian jusqu'à la première et la deuxième évaluation tumorale dans la cohorte. Résultats : Parmi les 320 patients de la cohorte COBEQOL, 229 ont été analysés. La survenue d'une détérioration définitive du Performance Status avant la première évaluation tumorale était significativement associée à la survie globale en analyses univariées et multivariées (HR 3.81, IC95% 1.76-8.27, p0.0001), ainsi que le Fonctionnement Physique au diagnostic 84 (HR 1.71, IC95% 1.16-2.52, p=0.007), l'âge 50 ans, les leucocytes 8,5 G/L, les LDH 225 UI/L, et l'albumine

Nevertheless, its development has been very slow, and its ultimate potential is today somewhat in doubt. In doubt because it does not cure any cancer patients except a) females carrying placental choriocarcinoma, a semi-allogenic tumor, in which case, cure may be in fluenced by immune rejection, and b) children suffering from Burkitt's tumor, where the probable reason for the cure is that all the neoplastic cells are in the cycle, which is a unique condition among all the human tumor varieties. Whether the long term survivors in acute leukemia, lymphomas, certain sarcomas and certain testicular tumors are "cures" will require longer follow-up. The idea that chemotherapy does not cure most cancer patients because all their neoplastic cells are not in cycle has led to the use of drug combinations. Whatever they are, "cocktail combinations" which are made up of drugs given according to any timing, or scientific combinations, based on pharmacodynamics, pharmaco kinetics or cell kinetics data, are more toxic than single drugs, and are all the more toxic as the number of drugs in the combination is increased.
