Nursing educators are increasingly utilizing high-fidelity simulation to provide realistic educational experiences to prepare students to enter the workforce. High-fidelity simulation is utilized with the belief when students encounter real-life patients they will be more confident in their ability to care for them. This quantitative study examined the effects of high-fidelity simulation on senior nursing students perceived self-efficacy and perceived practice readiness. The sample (N=48) consisted of senior associate degree nursing students at a community college in the Southeast. The findings showed high-fidelity simulation had a positive effect on students perceived self-efficacy and perceived practice readiness at certain points in the study, while other times not demonstrating a positive correlation. Implications for use of simulation as a teaching strategy in nursing education and recommendations for future research were proposed.

Today's health care environment requires competent staff that can care for patients' consistently changing conditions. High fidelity simulation is important to nurse clinical educators as an alternate method to provide experiences that assimilate, expand, and enhance their self-confidence. National League for Nursing states that nurses' self-confidence improves the performance level in life saving conditions (National League for Nursing, 2013). The problem statement of this Direct Practice Improvement (DPI) project was the following: It is not known to what extent using high fidelity simulation in adult cardiac life support (ACLS) renewal courses increase the self-confidence of nursing staff. The purpose of this quantitative project was to determine if high fidelity simulation increases nurses' confidence level while performing ACLS. The clinical question was the following: Does using high fidelity simulation in ACLS renewal classes increase nursing staff's confidence level and improve their performance at work? The theoretical foundation for this project incorporated Benner's Novice to Expert model. The design of this DPI project was a pre- and post- survey. A paired t-test was used to analyze the data. The population was 64 registered nurses who self-enrolled for an ACLS class using simulation. Statistical findings of this project suggest that high fidelity simulation increases the self-confidence level of the nurses.

Background: Advance care planning (ACP) conversations aim to ensure the articulation and documentation of patients' health care goals. These conversations should occur early and longitudinally. However, they are often not done or not done iteratively. Clinicians lack training and knowledge in ACP to have effective conversations. Therefore, a simulation was developed and tested among registered nurses (RN). Aim: This study aimed to examine the feasibility of an ACP simulation called Conversations Had at Trying Times (CHATT) among RNs, and to explore changes in knowledge, attitudes, and self-efficacy. Theoretical Framework: A framework was developed with the National League of Nursing Jeffries Simulation Theory and Bandura's Self-Efficacy Theory. Method: The study consisted of two phases: a) simulation development, and b) simulation testing. Construct and content validity guided the development of the simulation using DeVellis's (2017) scale development guidelines and the International Association of Clinical Simulation in Learning Standards of Best Practice: Simulation DesignSM. Simulation testing consisted of a pilot one-group pre/posttest design. The Caring Efficacy Scale and the Advance Care Planning Knowledge, Attitudes, and Practice Behaviors Scale were used to assess knowledge, attitudes, and self-efficacy among RNs. Evaluation of the simulation was done with the Simulation Design Scale and Student Satisfaction and Self Confidence in Learning Scale. Data analysis consisted of statistical analysis of mean change from pre-to-post simulation, and calculations of correlations between variables and demographics. Results: A researcher-designed high-fidelity simulation enhanced nurses' skills with CHATT. Thirty-six RNs completed the simulation in groups of four to eight in six simulation sessions. Knowledge, attitudes, and self-efficacy increased pre-to posttest. Evaluations post-simulation confirmed the feasibility of the simulation. The largest variation in results was found among the scores and experiences of the graduate nurse resident groups who worked as RNs for less than one year. Significance and Conclusions: This is the first high-fidelity simulation that was developed and validated for RN education in ACP conversations. Newer nurses with less than one year of experience had the lowest scores, which suggests a need for professional development in hospice and palliative care, curriculum development among nursing schools, and strong mentorship in caring for seriously ill patients.

New nurse graduates may be ill-prepared to care for multiple patients in the acute care setting due to limited clinical experience. Nurse educators must find ways of bridging the theory-to-practice gap. High-fidelity simulation is one mechanism for bridging this gap. This pilot study explored the effect of multi-patient simulation (MPS) on students' self-efficacy and perceived clinical judgment. This quasi-experimental study sought to explore the readiness of BSN students to care for multiple patients through the use of MPS and a self-efficacy survey measuring perceived level of confidence (LOC) in clinical care. Findings were inconclusive regarding whether MPS as a teaching method helped nursing students increase their perceived LOC and improve clinical judgment skills. Students agreed that the design should be part of nursing education and it identified gaps in their knowledge. This study raised an awareness of what students lack in terms of prioritization management when caring for multiple patients.
