De nos jours, les fonnes sévères d’usure dentaire se rencontrent plus fréquenunent mais aussi plus précocement. Parallèlement, l’espérance de vie de l’homme ne cesse d’augmenter. L’étiologie multifactorielle de ces lésions rend leur thérapeutique complexe; celle-ci doit être restauratrice, mais aussi étiologique, pour éviter toute récidive. Face à la durée de vie limitée des prothèses et à des thérapeutiques prothétiques fixes conventionnelles trop invasives, il en résulte une inquiétude et une réflexion quant à la meilleure stratégie thérapeutiqueà adopter: dépulper ou conserver la vitalité pulpaire? Même si aucun consensus n’existe, considérant le coût biologique qu’occasionnent à la fois le principe de dépulpation pré-prothétique et celui de préparation périphérique, la prothèse fixe collée permet de restaurer en conservant, plutôt que de restaurer en mutilant. Sur une dent qui par sa faible hauteur offre peu de rétention mécanique, la réalisation d’une restauration indirecte collée représente une réelle alternative aux restaurations prothétiques fixes scellées conventionnelles. Ainsi, dans le cas d’usure dentaire sévère, et s’inscrivant dans une logique contemporaine d’économie tissulaire, le choix du collage à condition toutefois d’en respecter scrupuleusement les protocoles et les impératifs, devrait prendre le pas sur des thérapeutiques plus délabrantes.
