Contexte : les écrans ont envahi notre quotidien. Cette exposition massive n'est pas sans conséquences sur le développement de nos enfants puisqu'elle affecte le sommeil, l'attention, les résultats scolaires et diminue les activités sportives. De nombreuses campagnes de prévention ont été mise en place à différentes échelles pour informer les enfants et leurs parents des risques. Notamment la « semaine sans écran » au sein des établissements scolaires. Mais aucune n'a été évaluée afin de voir si elles modifiaient les comportements. Nous avons cherché à savoir si cette semaine diminuait le temps passé devant les écrans via une étude quasi-expérimentale avant-après. Matériels et méthodes : nous avons questionné les parents d'élèves sur le temps de consommation d'écran de leurs enfants avant et après la « semaine sans écrans ». L'intervention consistait à inciter les enfants à ne pas consommer d'écrans et à informer leurs familles des risques liés à une consommation trop importante. Résultats : au total 247 élèves ont reçu l'intervention. 75% des enfants regardent entre 30min et 2h d'écran passif par jour les jours de classe. 13.5% regardent plus de 3h d'écrans passifs les jours de classe. Les week-ends et jours de vacances, le nombre d'enfants consommant plus de 3h par jour augmente passant à 50%. Au terme de l'étude, nous n'avons pas trouvé une diminution statistiquement significative du temps passé devant les écrans après notre intervention sauf pour le temps passé devant les écrans passifs le week-end. En effet pour les grands consommateurs d'écran, il y a une réduction de 14.3% de la consommation (p=0.0192). Il n'y a pas non plus de différence avant et après l'intervention en ce qui concerne les durées de sommeil ou l'activité physique. Discussion : nous n'avons pas réussi à démontrer une diminution du temps passé devant les écrans suite à l'intervention par manque de puissance et par lacune méthodologique. Une intervention ponctuelle est suffisante pour informer les parents et les enfants, mais insuffisante pour modifier dans la durée leurs comportements. Notre étude reste originale car aucune « semaine sans écrans » n'a jamais été évaluée auparavant. D'autres études ont évalué les actions de prévention et ont démontré la nécessité de mettre en place des interventions longues et pérennes.

Dans notre société devenue hyper connectée, on peut observer que les écrans prennent une place de plus en plus importante dans la vie des enfants, avec un temps passé à les utiliser qui ne cesse de croître. Parallèlement à ce phénomène, on note une augmentation du diagnostic du trouble de l’Attention avec ou sans Hyperactivité (TDAH). De nombreux scientifiques s’interrogent sur l’effet des changements de notre mode de vie et notamment du temps passé à utiliser les écrans sur l’apparition de ce trouble. Les données de la littérature actuelle montrent qu’il existe une association entre temps passé à utiliser un écran et la survenue de TDAH. L’objectif principal de cette étude est de comparer le temps passé à utiliser un écran chez les enfants atteints et indemnes de TDAH. Méthodes : nous avons réalisé une étude transversale, en distribuant un questionnaire à destination des parents dans trois écoles élémentaires de la région de Nîmes. Le critère de jugement principal était l’évaluation du temps passé devant un écran, tous types d’écrans confondus. Résultats : entre le 21 janvier 2019 et le 1er février 2019, 456 élèves de 6 à 12 ans ont été inclus. Concernant l’analyse du critère de jugement principal, la moyenne journalière du temps passé devant un écran pour le groupe TDAH était de 120,7 min/j et pour le groupe non-TDAH de 155,3 min/j, il n’y avait pas de différence statistiquement significative entre ces deux groupes en analyse univariée. Dans l’analyse multivariée ajustée sur le sexe et le taux de redoublement, on retrouve que les enfants sans TDAH ont une tendance statistique à passer plus de temps devant les écrans que les enfants atteints de TDAH. Conclusion : cette étude a permis de montrer qu’il n’y a pas de différence entre le temps global passé devant un écran entre des enfants atteints de TDAH et des enfants non atteints de TDAH scolarisés en élémentaire en analyse univariée. Il est toutefois intéressant de noter que les enfants indemnes de TDAH ont une tendance à passer plus de temps à utiliser les écrans que les enfants ayant un TDAH en analyse multivariée.

Jean-Baptiste Say (1767–1832) was one of the first great economists to have laid down the foundations of economic science.
