Le traitement des fiircations dans les relations endo-parodontales, concerne tous les praticiens qui y sont un jour confrontés En effet, la prévalence de l’atteinte des fiircations est de 13,7% des individus. C’est pourquoi, il faut bien connaître la grande diversité anatomique que représente les fiircations ainsi que les racines qui leurs sônt associées. La zone des fiircations est en relation avec le parodonte et 1’ endodonte, qui sont deux systèmes étroitement liés de par leurs multiples communications, d’où des étiologies qui peuvent être parodontale, endodontique ou endo-parodontale. La connaissance de ces étiologies permet de fhire un examen clinique approprié, afin d’établir le diagnostic, par une classification des lésions ifircatoires. Une fois le diagnostic réalisé, un plan de traitement approprié pourra être appliqué. Les lésions- de classes I se traiteront classiquement par un détartrage et surfaçage. Pour les classes II, des études comparatives entre les différentes techniques de régénération parodontale sont menées. II en ressort que toutes ces techniques offitnt de bons résultats, surtout pour le traitement combiné (comblement osseux et pose d’une membrane) ; mais qu’ils ne sont pas prédictibles Pour les classes III, ces techniques de régénération ne sont pas efficaces. Dans le cas de lésions endo-parodontales, l’obturation hermétique endocanalaire conventionnelle ainsi que le scellement du plancher pulpaire sont suffisants pour résoudre l’étiologie endodontique; si un stripping était en cause, le mineraI trioxide aggregate semblerait actuellement le matériau le plus indiqué.
