Depuis les travaux de recherche de Petrovic et MacNamara, il est régulièrement prescrit un activateur afin de modifier la direction et la quantité de croissance ; Fränckel définit ce concept sous le terme d' "orthopédie fonctionnelle". Face à des résultats cliniques satisfaisants mais parfois aléatoires, deux problématiques reviennent sans cesse : Les effets obtenus sont-ils squelettiques ou alvéolaires ? Cette réponse ayant un impact sur la stabilité des résultats. Les effets sont-ils le résultat du port de l'appareil ou plutôt de la croissance cranio-faciale normale ? Cette réponse pouvant amener à se poser la question de l'intérêt des traitements précoces. Afin de répondre à la première question, nous proposons une étude sur 30 patients présentant une dysmorphose de classe II en période croissance et traités par le Twin block de Clark. Pour chaque patients deux téléradiographies (une de départ et une de fin avec un écart moyen de 20 mois) seront analysés à l'aide de mesures céphalométriques conventionnelles ainsi qu'une méthode de superposition strucurale (analyse LDV). Pour résoudre la deuxième problématique, nous confronterons nos résultats à ceux d'un échantillon témoin de 30 patients présentant des variables de départ comparables au groupe traité. Bien que les modifications engendrées par l'activateur soient qualitativement positives (réponse basale majoritaire), elles restent médiocres en quantité, bien inférieures aux résultats escomptés ainsi que ceux publiés dans la littérature orthodontique. Le facteur récidive n'étant pas toujours pris en compte et les cas présentés problablement sélectionnés. Une question reste en suspens ; les bénéfices obtenus par le traitement précoce des classes II à l'aide d'un activateur sont-ils une réalité ou une illusion ?
