Bile acids are increasingly being seen as extremely important carcinogenic agents in cancers of the bile duct, liver, colon, rectum, and oesophagus. They are essential agents involved in lipid digestion and absorption in mammals, however, they also play wide-ranging roles in a variety of disease states ranging from diabetes to cancer. They have evolved exquisite mechanisms for controlling their own synthesis and to ensure that they are produced at correct concentrations and also kept in the correct anatomical environment. It is only when these fine levels of controls are breached that Bile acids become associated with disease. This breaching of control mechanisms can occur through dietary means (e.g. in obesity) whereby excessive levels of Bile acids are produced and converted (via the bacterial flora) to damaging secondary Bile acids. Furthermore, lack of re-absorption of Bile acids can lead to liver pathologies. The atypical movement of Bile acids into the oesophagus, stimulated by episodes of reflux, is linked to oesophageal cancer. In recent years there have been tremendous advances in the understanding of the mechanisms behind the toxicity and bioactivity of different Bile acids and these are covered at length in this book. Prior to the publication of this book there was no single source of information on the toxicology and bioactivity properties of Bile acids. The book also uniquely collects all the relevant information together regarding the role of Bile acids in human disease and the mechanisms underlying Bile acid induced pathology. The first two volumes of this series addressed themselves to the chemistry, physiology, and metabolism of the bile acids. The present volume is devoted to the pathophysiology of bile acids. As the role of bile acids in health and disease is being increasingly recognized, we have chosen for discussion a wide range of topics of current importance. The presence of bile acids in brain tissue and their possible role in demyelinating diseases form the subject of a provocative discussion. As an extension of this theme, the presence and quantification of bile acids in extrahepatic tissues is the subject of one chapter. The pathophysiological implications of bile acids at the macromolecular level is highlighted by a chapter on the influence of bile salts on the activity of various enzymes. The general area of hepatobiliary diseases is discussed in two chapters: one describes changes in bile salt metabolism in liver diseases and the other focuses on cholesterol gallstones and their formation and dissolution. Over a decade has elapsed since the last volume in this series was published. At that time we considered that we had comprehensively covered all aspects relating to bile acid chemistry and physiology. The initial chapter discusses techniques of extraction and isolation of bile acids from biological fluids. It is followed by descriptions of physical methods of analysis and discussions of the way these techniques have been applied in the field of bile acid research. Of practical value is the inclusion of a comprehensive list of spectra obtained for refer ences by nuclear magnetic resonance spectroscopy and mass spectrometry . These chapters are followed by reviews of biological methods of immuno assay and bioluminescence. Beginning in 1970, the International Bile Acid Meeting has taken place every two years and each time new progress in our understanding of the complex role of bile acids in many metabolic processes of the liver and the intestine has been revealed by a selected group of leading scientists from all over the world. Although originally mainly physiological data on bile acid synthesis and transport were emphasized, and later on also the therapeutic benefit of bile acids in gallstone disease and cholestasis was discovered, we have come now to the molecular biology and genetic era with major discoveries in transport defects and related diseases. This book is the proceedings of Falk Symposium No. 120, held in The Hague, The Netherlands, on October 12-13, 2000 - the 16th International Bile Acid Meeting. One of the main discoveries recently has been the identification of nuclear receptors for bile acids, which gives them a much broader perspective than previously anticipated. It even suggests that bile acids can regulate their biosynthesis and enterohepatic circulation transcriptionally. It will therefore not be surprising that this topic, together with the molecular regulation of cholesterol 7alpha-hydroxylase and cholesterol homeostasis, has a dominant place in these proceedings. Another important topic is the progress in our molecular understanding of hepatic (both at the basolateral and canalicular sites), cholangiocytic and intestinal bile acid transport processes. Further insights into genetic defects causing cholestasis or intestinal malabsorption in animal models and in human diseases are also discussed by a number of well-known authors. Finally the last section deals with new findings on the role of bile acid therapy in cholestatic syndromes or chemoprevention and with the potential benefit of bile acid inhibitors. All contributors provide an update on the most recent developments in their field.
