Introduction : Le frottis cervico-utérin (FCU) est actuellement l'outil de dépistage de référence du cancer du col de l'utérus (CCU). Sa généralisation a permis une nette régression de l'incidence et du taux de mortalité du CCU, mais il subsiste des inégalités de dépistage en France. L'objectif de cette étude était d'évaluer l'influence du sexe du médecin généraliste (MG) et de sa pratique sur le taux de participation au dépistage du CCU. Méthodes : Étude épidémiologique descriptive réalisée sur 2 périodes du 1er mars 2015 au 23 janvier 2017 auprès de 343 MG rattachés à la Caisse Primaire d'Assurance Maladie (CPAM) des Flandres. Un sondage téléphonique chez les médecins inclus a pwwwwwwwwwwwwwwwwwermis de statuer sur la pratique par le médecin du FCU. La base de données de la CPAM a fourni les caractéristiques des MG (sexe, nombre de patientes âgées de 25-65ans et celles ayant un remboursement de FCU, cotation du FCU). Elle a été enrichie par l'indice de défavorisation européen (EDI) et l'offre de soins gynécologiques de l'aire d'installation du MG. Résultats : Le taux de réponse des MG interrogés était de 98,6%. Le taux de participation moyen au dépistage du CCU était de 50,1 %. Ce dernier était plus élevé si le MG était une femme (p=0,0011), qui réalisait (p=0,0058) et cotait les frottis (p=0,0009). Toutefois, cette association n'était plus vérifiée en analyse multivariée pour le sexe du médecin (p=0,0530) et la pratique du FCU (p=0,0822), mais restait significative sur la cotation du frottis (p=0,0007). Conclusion : Le sexe du MG n'influençait pas le taux de dépistage du CCU par FCU de sa patientèle alors que la cotation de l'acte était associée à un meilleur taux de dépistage. Une plus forte implication des MG dans la stratégie de dépistage du CCU par le FCU ou d'autres méthodes (test HPV etc.) est primordiale pour réduire les inégalités de santé.
