Challenges in the delivery of health care services to American Indian people includes the recognition of the importance of cultural competency training as a part of the educational programs of study for each health care provider. The acquisition of cultural competency potentially could increase a health care provider’s ability and openness to engage American Indian patients, increase quality of health care and positive outcomes, and reduce health disparities. It is important as a survival strategy for American Indian people to ask for health care providers to acculturate to the American Indian culture. A non-experimental, cross-sectional, correlational study will be conducted to explore the health care providers awareness of American Indian culture and patient satisfaction with cultural competence of health care providers for American Indian patients of the Indian Health Clinics in southern California. Research study outcomes could be useful to other American Indian patients of Indian Health Clinics throughout the United States. A convenience sample of health care providers and American Indian patients will be recruited at Indian Health Clinics for research to study this topic. Data will be analyzed using descriptive statics and correlations. Finding will be disseminated widely to inform the practice of health care providers caring for American Indian people.

OBJECTIVES: Significant health and health care disparities of American Indians (AIs) have been well documented. Implementing cultural competency programming into the health care setting has been shown to be an effective method to address these disparities. Through a detailed review of the relevant literature, the researcher described definitions and methods of cultural competence, historical and contemporary experiences of AIs in California that contribute to their below the standard health status, and frameworks for establishing systems and services to address the disparities. METHODS: The researcher reviewed academic, organizational and government publications concentrating on the historical and contemporary experiences of AIs in the U.S., with emphasis in California; the health care system obstacles impeding racial and ethnic populations' access to adequate health care; cultural competence and other interventions that address the obstacles to care. He also examined the data of the few studies that identify information to include in cultural competency programming in the region for AIs. The researcher has extensive experience working in the field of AI health care and has served Tribal communities in this capacity for 13 years. Work in this field coupled with the researcher's life-long learning of traditional knowledge from traditional knowledge holders contribute to information in this paper. RESULTS: Definitions and methods of cultural competency, relevant experiences of AIs in California contributing to their health disparities, cultural components and other strategies to address the disparities were researched, identified, examined and described. Health care obstacles to care were identified at the National, State, County and clinical levels. A framework of AI cultural competence interventions surfaced, including development of culturally-appropriate health care services, health care provider training on relevant cross-cultural issues and culturally-appropriate community capacity enhancement strategies. These and other strategies can assist in remedying AI health disparities in California. CONCLUSIONS: Demographic changes of the AI population over the next 10 to 15 years and persistent low health status of this group underscore the need to remedy AI health disparities. Incorporation and maintenance of culturally competent systemic and clinical programming can improve care for this population and reduce these disparities.
