Excerpt from Skin and Venereal Diseases, Vol. 10 The growing tendency to take into account the nonspecific results of syphilis and tuberculosis, from the diathetic standpoint, is manifest in many recent discussions of the dermatoses. W. A. Hackett and N. E. Aronstam classify the complementary dermatoses of syphilis into: First, morbid conditions of the dermic glandular appendages which are divisible into (a) anomalies of function, (b) structure anomalies; second, inflammatory states of the skin; third, cutaneous neuroses; fourth, undefined ephemeral dermatoses. The perspiratory glands are never affected to any great extent. The sebaceous glands bear the brunt of the process and exhibit either secretion derangement or actual pathologic states. Seborrhea sicca is more prevalent than seborrhea oleosa and may manifest itself years after when all syphilis has been eradicated. The duct of a gland is more frequently affected by pathologic change than the racemose extremity. Acne results; very intractable in treatment which recrudesces after temporary abatement and is attended by severe subjective symptoms. It is apt to occur during secondary defervescence, is not influenced by specific treatment, but yields to the internal administration of ichthyol and its local use with adrenalin solution. Had been separated from her husband, a dissolute man, for several years never had intercourse with any other man. Had sore throat seven years ago and at intervals afterward. For last three years her throat had troubled her greatly, becoming, in spite of treatment, steadily worse. On ex amination the whole of her soft palate was seen to be gone, also a triangular piece of the hard palate about 3 cm. (one and one fifth inches) wide at the base, the loss of substance extending forward to within 3 cm. (one and one fifth inches) of the teeth. J. M. Miller' in a ten-year-old girl who had had measles, varicella, pertussis and tonsillitis. With the pertussis, buccal, nasal and aural hemorrhage occurred. The tonsillitis was followed by pain in the hands and feet. The present attack made its onset with chill, headache, and sore throat, followed by nausea and vomiting. Swelling and redness of the wrists, hands and ankles, then appeared followed by the state before mentioned. Erythema occurred after 61 tonsillotomies and adenoid removals by Wingrave.' The septic element could be ex cluded. The temperature never rose above The age limit was 14 months to 23 years. There were forty-five females and sixteen males. The eruption, which was poly morphous in type, did not desquamate and was chiefly on the head and back. It appeared on the second or third day after the operation and lasted from two to four days. Adamson.4 The patient, an attractive young woman, was not suspected by Adamson of hysteria. Her illness began with the unexplained appearance of patches of weeping eczema on the face. These continued to recur on the cheeks, forehead, neck, arms, and later thighs and legs despite treatment and covering with dressing. The crop never failed for a year, the condition being some times better, sometimes worse. In the initial stage there was an erythemous flush which rapidly became moist, suppurated and healed with a crust. The eruption was often square or linear, sometimes round. A dermatol ogist consulted pronounced the case one of dermatitis herpetiformis. The patient went to a specialist without results. Another dermatologist regarded it as a case of dermatitis artefacta. The parents were skeptical; watch ing by a nurse failed. The patient was removed at length to a nursing home. Here three days after admission, a movement under the bedclothes led to the detection of the lady's hand holding most unsuspiciously a handkerchief, but in the handkerchief was a small ragged piece of pumice-stone! Thereupon this protracted dermatosis came quickly to a complete end. The patient later feigned hemoptysis and bowel obstruction, and was detected.
