Devant un vieillissement de la population et une espérance de vie augmentée dans un contexte polypathologique, les Établissements d'Hébergement pour Personnes Agées Dépendantes (EHPAD) occupent une place de plus en plus importante dans les structures de soins et de vie pour le secteur gériatrique. Face à la complexité du circuit du médicament en son sein, notamment de par les nombreux acteurs de santé intervenant, sa sécurisation devient primordiale et représente un des objectifs de santé publique prioritaires. En effet, l'iatrogénie médicamenteuse serait responsable de plus de 10 % des hospitalisations chez le sujet âgé, dont près d'un tiers serait lié à une erreur d'administration. Cela peut être dû à une mauvaise utilisation du médicament, plus particulièrement par la modification de la galénique des formes orales solides (comprimés et gélules) effectuée par le personnel soignant devant des patients ayant des difficultés de déglutition (plus d'un tiers des résidents par établissement concerné). Mal informé des nombreux risques que cet acte comporte, ce dernier en a fait une pratique banalisée, le plus souvent sans en informer le prescripteur ou le pharmacien. Devant ce constat, un état des lieux des pratiques a été établi dans quatre EHPAD de l'Isère, ainsi qu'une formation du personnel soignant sur ce sujet, afin d'éviter cette pratique au maximum, ou tout du moins de la contrôler.

Différentes pistes seront abordés, aussi bien à l'étape de prescription bien que purement médical, à l'étape de dispensation qui est un acte pharmaceutique, ainsi qu'à l'administration du médicament au résident. Dans tous ces cas le rôle du pharmacien sera souligné par la nécessité de s'impliquer davantage dans le circuit du médicament en EHPAD dans une démarche de qualité et de sécurité de son acte pharmaceutique en privilégiant la proximité avec l'équipe médicale et le patient.

Les EHPAD sont des Etablissements d'Hébergement des Personnes Agées Dépendantes. Ils ont été créés pour faire face à l'indispensable médicalisation des maisons de retraite. Ces établissements médico-sociaux doivent, tout comme les établissements sanitaires, garantir une sécurisation optimale de la PECM de ses résidents. Le pharmacien choisi par l'EHPAD a la responsabilité, tandis qu'il est situé hors de l'établissement, d'assurer une dispensation de qualité. Afin d'optimiser la sécurisation du circuit du médicament, le pharmacien a recours à la préparation des doses à administrer (PDA). Bien que les textes juridiques existants ne l'autorisent pas clairement, à ce jour aucune loi n'interdit la pratique de la PDA. Dans le projet "maison de retraite " et afin d'apporter un service de qualité et une sécurité optimale du circuit du médicament, la pharmacie Esquirol s'est équipée d'un robot de PDA permettant de préparer de manière automatisée des piluliers pour les résidents des maisons de retraite. A l'occasion du stage de 6ème année effectué au sein de la pharmacie, j'ai pris conscience du rôle du pharmacien et de son avenir au sein des structures de type EHPAD. La PDA apporte un plus reconnu et fait du pharmacien un acteur de Santé Publique.

Evaluates the carcinogenic risks to humans posed by the use of four antiretroviral agents four DNA topoisomerase II inhibitors used in the treatment of cancer and an additional three pharmaceutical agents (hydroxyures phenolphthalein and vitamin K substances). The volume marks the first IARC evaluation of nucleoside analogs that act as antiviral agents. The evaluation responds in part to recent findings that zidovudine (AZT) an effective antiretroviral agent now being given to pregnant HIV-infected women to prevent maternal-to-fetal transmission of the virus is a transplacental carcinogen in mice. The opening monograph evaluates the carcinogenicity to humans of the antiretroviral nucleoside analogs zidovudine (AZT) zalcitabine (ddC) and didanosine (ddI) and the antiherpesvirus drug aciclovir. Of these aciclovir and didanosine could not be classified on the basis of available data. For zidovudine transplacental administration to mice resulted in an increased incidence and multiplicity of lung and liver tumours and in an increased incidence of female reproductive tract tumours in one study but not in another involving treatment at a lower dose. Despite observation of toxic effects in some studies of humans human carcinogenicity data were judged to provide inadequate evidence of carcinogenicity in humans. Zidovudine was classified as possibly carcinogenic to humans. Similar weaknesses in human carcinogenicity data for zalcitabine which consistently induces thymic lymphomas in mice resulted in its classification as possibly carcinogenic to humans. The second monograph evaluates four DNA topoisomerase II inhibitors: etoposide teniposide mitoxantrone and amsacrine. Of these etoposide - one of the most widely used and effective cytotoxic drugs in combination therapy - was classified as probably carcinogenic to humans and etoposide in combination with cisplatin and bleomycin was judged to be carcinogenic to humans. Teniposide was classified as probably carcinogenic to humans and mitoxantrone and amsacrine were classified as possibly carcinogenic to humans. Of the three pharmaceutical agents evaluated in the final monograph hydroxyurea which is widely used in cancer treatment and increasingly in combination with didanosine in HIV infection could not be classified. Phenolphthalein a widely used laxative now being withdrawn from the market in many countries because of toxicological concerns was classified as possibly carcinogenic. Vitamin K substances could not be classified on the basis of available evidence.
