Background :The placement of immediate implants in the posterior sector is a widespread procedure where the success and survival rates are similar to those of traditional protocols. However, large extraction sockets are difficult to seal without mucoperiosteal flap reflection, an implant preparation sequence that considerably deviates from typical manufacturer protocols, crestal socket morphology not amenable to standard healing abutment placement and the possibility of high occlusal forces with inadvertent function when a provisional implant is planned.Aim/Hypothesis : The purpose of this case series are to assess related technique that will allow chairside manufacturing of custom healing abutments as also called the anatomic healing abutment for immediate molar placement.Materials and Methods A 55-year-old nonsmoking male patient, with no systemic condition and a 42 year female healthy with no significant medical history patient had lost her lower first molars due to nonrestorable extensive decay. According to clinical and radiographic examination on the patient detecting decayed remaining roots in molar tooth and performed an extensive restoration presenting deep, subgingival distal decay on tooth. So that, we decided to extract related tooth and then perform immediate implant placement. After minimally traumatic tooth extractions, the alveolus was carefully cleaned and washed surgically with a saline solution, and dental implants (iRES SAGL, Switzerland) was placed in the center of the interradicular bone, in a type A position according to the Tarnow classification. After then a temporary abutment was fabricated utilizing flowable composite, which adapted to the socket architecture, to serve as a customized healing screw. It is called the anatomic or customised healing abutment. This technique preserves the anatomic emergence form with sutureless implant site sealing and improves the predictability of final restoration fabrication and delivery. After 8 weeks of the Osseo integrated period, the custom healing screw served successfully as a tooth profile for peri-implant tissue. Results : Uneventful healing was observed durig this time period in this cases. It is important to note that the initial contour appeared to have been maintained. The anatomic harmony abutment ultimately allows for more predictable final restorations by providing the dental laboratory a restorative profile. thus idealizing the final restoration. The soft tissue margin was stable and the esthetic appearance of the implant site was well maintained and with no clinical signs of peri-implant inflammation and bleeding on probing. Conclusions and Clinical Implications : Implants may be placed in ideal restorative positions at the time of mandibular molar extraction. With the anatomic harmony abutment technique, the soft tissues upon maturation closely resemble the natural root contour, which allows for the generation of esthetic and functional implant-supported restorations.

Background: Despite the ongoing debate about the meaning of keratinized gingiva, most papers agree that a sufficiently broad band of keratinized tissue exert a positive effect on long-term prognosis and maintain of dental implants.The recent introduction of xenogenic collagen matrixes further expanded the clinical alternatives in the management of soft tissues healing during pre-implant or peri-implant regenerative procedures.Aim: The aim of this prospective cohort study is to test the performance of a new xenogenic collagen matrix (Mucodermu00ae, Botiss Dental, Germany) as a socket sealing material, to allow second-intention healing of post-extractive sockets filled with a xenogenic bone substitute or with an immediate submerged implant.Materials and Methods: 10 patients (4 males and 6 females - mean age: 53.2 years) were recruited, presenting with a single-rooted tooth scheduled for extraction, because of unsalvageable endo-perio lesions or vertical root fractures.After atraumatic tooth removal, the post-extractive alveolus received either a socket preservation procedure or an immediate submerged implant. Demineralized bovine bone mineral integrated in a 10% collagen matrix (Bio-Oss Collagenu00ae, Geistlich Pharma, Switzerland) was used as a socket preservation material or to fill the gap between the implant surface and the bony walls of the alveolus.In both cases, the gingival margins of the alveolus were sealed with a xenogenic collagen matrix (Mucodermu00ae, Botiss Dental, Germany). Through 5/0 monofilament sutures, the matrix was gently stabilized subperiosteally under the palatal and buccal gingival margins of the socket for 2/3 of its surface, whether 1/3 remained exposed for second intention healing.The following parameters were evaluated: a) exposed surface of the matrix at the end of surgery (T0);b) soft tissue healing at 1, 4, 6, and 8 weeks from surgery (T1-4);c) aesthetic performance provided by the socket sealing material, quantified deriving the colorimetric score u2206E between the regenerated site and the surrounding gingiva, 8 weeks after surgery (T4);d) histological aspect of gingiva samples, harvested 20 weeks after surgery (T5) from the regenerated area.Results: a) the mean postoperative exposure area of the matrix was 26,25 mm2 (14,2 to 38,84 mm2); b) 8 weeks after surgery (T4), full wound closure was achieved in 9 out of 10 sites with healthy keratinized tissue. A single patient (#3) did not achieve full wound closure at T4, starting from the highest postoperative matrix exposure rate of the cohort (38,84 mm2); c) the mean colorimetric score u2206E between the regenerated site and the surrounding gingiva at T4 was 3,76 (3 to 6,55).
