Despite years of availability, the polymyxin drugs are becoming more important options for treatment of gram-negative infections. Expanded discussion and dosing regimens for Polymyxin B and Polymyxin E (Colistin) are included. - There is increasing clinical application of continuous, or prolonged, infusion of antibacterials for those drugs where optimal efficacy correlates with time above MIC of the target bacteria: cefepime, ceftazidime, doripenem, meropenem, piperacillin-tazobactam. A new table (10D in print) in all editions suggests regimens for continuous or prolonged infusion of these agents. - Hepatitis C (HCV) infects over 175 million people worldwide.
