The researcher examined the transition from high school to adulthood for people with moderate to severe disabilities. The researcher posed the question, "What can parents do to assist their child with moderate to severe disabilities in the transition from high school to adulthood?" The purpose of the project included the assembly, design and production of a manual for parents to support them in assisting their child with a moderate to severe disability before, during, and after the transition to adulthood, in part through accessing the local regional and educational resources and services available in North County San Diego, CA. Activities are provided for the parents to utilize at home with their child to facilitate the transition process. The three areas of the Individual Transition Plan (ITP), which include education or training, employment, and independent living, as required by law, were addressed in the manual for parents. The method employed for creating the project were to examine current research, then determine areas of unmet needs, and finally to produce the transition guide for parents. The resources determined to be most relevant to parents of children with moderate to severe disabilities were compiled and incorporated into the guide. The most relevant topics formulated the sections in the manual. The intended audience for the manual is parents of a child or children with moderate to severe disabilities in high school, however, Education Specialists, Counselors and other educators are encouraged to utilize this manual and to distribute to parents in their classrooms.

The purpose of this project is to prepare high school students with developmental disabilities to advance from the high school classroom to an adult transition program. Present research demonstrates that students are frequently unprepared for community-based programs from current high school instruction. The current instruction in high school classrooms for students with developmental disabilities has concentrated on academic formats and objectives. In order for a high school student to be prepared in the transition process, best practice research indicates that increased planning with both students and parents is necessary. A curriculum manual entitled, Pre-Transition Manual, is presented to provide the high school teacher a structured transition planning and preparedness guide for teaching the many facets of the transition process from high school to an adult community-based program. The Pre-Transition Manual will provide instruction at the high school level for teacher, students, and parents throughout freshman, sophomore, junior, and senior year. The Manual will provide instruction for application procedures for community supports, functional goal setting, self-advocating, and heightened awareness of community based instruction. The application procedures include instruction to obtain ongoing services from San Diego Regional Center, developing documentation of personal medical information, California identification, and reduced fare transportation identification. Completing this process fosters the development of independent living skills, educational skills, and employment skills. This establishes clear planning and objectives for the Individualized Transition Plan, which promotes self-determination in an adult based community program and post secondary life after the age of 22.

The research answered the question, “What are evidence-based best practices and community resources for assisting in the transition of individuals with low incidence disabilities from high school and special education services to adulthood?” To answer this question, the researcher reviewed literature of evidence-based practices that increased success rates of students in special education after they transitioned into adulthood. A review of literature revealed that interagency collaboration, functional skill curriculum and assessment, vocational skill training, and self-determination awareness and training delivered to student between the ages of 16 and 22 positively influenced transition from special education services into adulthood. The community resources included in the manual are those accessible to North County San Diego residents.
