Le jeune médecin, titulaire d'une formation continue, doit promouvoir valeurs traditionnelles (patience, humilité) tout en faisant preuve d'ouverture. Avec le Référentiel Compétences et Métier en Médecine Générale, ces attentes définissent la pratique médicale d'aujourd'hui. En cela, elles sont une aide à l'autoévaluation et à l'enseignement, soulignant notamment la nécessité de formation en outils de communication.

Introduction: la médecine générale acteur essentiel du système de santé, aux rôles multiples et variés, fait face à de nombreux défis aussi bien au niveau des patients, des soignants que de l'organisation du système dans son ensemble. Le paradigme en santé plaçant le médecin généraliste, seul, au centre du dispositif n'a connu aucune évolution récente. La coopération entre professionnels de santé (« Skill mix » en anglais) a été initiée dans certains pays étrangers dès la seconde moitié du 20ème siècle. En 2020, en France, il n'existe pas d'équivalents. La perception des acteurs de soins primaires vis-à-vis du transfert de compétences en médecine générale en France est nécessaire. Matériaux et méthodes : étude observationnelle par méthode qualitative avec approche par théorisation ancrée et réalisée à partir de 9 entretiens semi-dirigés en soins primaires d'infirmiers et de médecins généralistes. Résultats: les professionnels de santé trouvaient important d'être soignants ensemble pour collaborer en santé dans un climat de confiance et partage. Ceci devait être supporté par une meilleure organisation du système actuel. Ceci était vu comme une révolution et une ouverture face aux conservatismes, pour lutter contre la souffrance professionnelle, l'inefficacité, et la déshumanisation dans les soins. Enfin il est apparu primordial et essentiel de toujours privilégier l'humain, au centre, aussi bien patient que soignant. Discussion: la problématique de souffrance dans les soins, aussi bien du côté des soignants que des soignés, était présente avec des retards pris dans l'organisation de la santé en soins primaires avec ses limites et menaces potentielles à venir. Enfin, le côté humain, primordial et essentiel a été discuté avec la notion de permaculture humaine, appliquée à la collaboration en santé, c'est-à-dire mieux soigner les hommes avec l'entraide humaine, de manière juste et durable.

In the early stages of planning the Third International Conference in System Science in Health Care, the steering committee members, most of whom had participated in the first conference in Paris (1976) and the second in Montreal (1980), made some basic decisions about organization of subject matter. The earlier meetings had been very successful in bringing together specialists from the health professions and the traditional sciences. Determinants of health - like physical activity levels and living conditions - have traditionally been the concern of public health and have not been linked closely to clinical practice. However, if standardized social and behavioral data can be incorporated into patient electronic health records (EHRs), those data can provide crucial information about factors that influence health and the effectiveness of treatment. Such information is useful for diagnosis, treatment choices, policy, health care system design, and innovations to improve health outcomes and reduce health care costs. Capturing Social and Behavioral Domains and Measures in Electronic Health Records: Phase 2 identifies domains and measures that capture the social determinants of health to inform the development of recommendations for the meaningful use of EHRs. This report is the second part of a two-part study. The Phase 1 report identified 17 domains for inclusion in EHRs. This report pinpoints 12 measures related to 11 of the initial domains and considers the implications of incorporating them into all EHRs. This book includes three chapters from the Phase 1 report in addition to the new Phase 2 material. Standardized use of EHRs that include social and behavioral domains could provide better patient care, improve population health, and enable more informative research. The recommendations of Capturing Social and Behavioral Domains and Measures in Electronic Health Records: Phase 2 will provide valuable information on which to base problem identification, clinical diagnoses, patient treatment, outcomes assessment, and population health measurement.
