Objectif : Apport de reconstruction millimétrique de la TEP numérique pour détecter les métastases en transit de mélanome primitif des membres, par rapport à la reconstruction standard et la reconstruction EARL. Matériel et Méthodes : Quarante-six examens acquis sur Vereos TEP/CT ont été reconstruit en (i) reconstruction standard, (ii) en reconstruction utilisant des voxels de 1mm3, (iii) en reconstruction EARL. Une comparaison de ces reconstructions a été réalisée par une relecture à l'aveugle pour évaluer la présence de métastase en transit et la qualité d'image, ainsi que le rapport Tumeur/Bruit de fond (T/B) et le bruit de fond dans les organes de référence. Résultats : Sept des trente-deux examens classés négatif en EARL sont positif en reconstructions 1mm3PSF, et 5 des 6 examens indéterminé sont positifs (P=0.01). En utilisant les reconstructions 1mm3PSF, 20 examens sont classés positif pour des métastases en transit dont quatre faux positifs. Vingt-quatre sont classés négatif, aucun d'entre n'a présenté de métastase en transit au cours des 17 mois.
