Selon l’étude de cohorte PAQUID, on estime à 1 million le nombre de personnes de plus de 65 ans atteintes de troubles neuro-cognitifs (TNC) majeurs en 2010 en France. Ce nombre s’élèverait en 2030 à 1 750 000. Ces patients sont hautement susceptibles de devenir dépendants concernant leur hygiène dentaire. Ainsi ils deviennent plus vulnérables quant à leur santé bucco-dentaire. La prise en charge de ces patients peut s’avérer complexe à un stade avancé de la maladie. Le parcours de soins des patients atteints de TNC majeurs reste cependant difficile à organiser à cause du manque de lisibilité des structures compétentes pour prendre en charge ces patients présentant des besoins spécifiques. L’objectif de cette thèse est de mettre en évidence les dispositifs et structures compétentes en Île-de-France pour proposer une prise en charge adaptée aux patients présentant des TNC majeurs. Nous détaillerons ainsi les conséquences des TNC majeurs sur la santé orale, puis préciserons les précautions de prise en charge bucco-dentaire des patients présentant des TNC majeurs. Enfin, nous explorerons les pistes de prise en charge en Île-de-France.

Le vieillissement de la population a pour effet d’augmenter de façon importante la prévalence de la maladie d’Alzheimer. Aujourd’hui, on estime à plus de 25 millions le nombre de personnes atteintes de la maladie d’Alzheimer dans le monde, et à plus de 860 000 en France. Les perspectives pour les années à venir sont inquiétantes, si rien ne vient à ralentir la tendance actuelle. En 2020, 1,2 million d’individus présenteront cette maladie en France. Sur le plan médical, une prise en charge pluridisciplinaire des patients atteints de la maladie s’impose. C’est pourquoi, tout professionnel de santé doit être dès à présent apte à prendre en charge ces patients, non seulement en secteur hospitalier, mais aussi au cabinet privé. Sur le plan bucco-dentaire, la prise en charge de ces patients s’impose aussi. En effet, il est acquis que maintenir une bonne hygiène orale permet non seulement de conserver une fonction masticatoire et une nutrition correcte, mais aussi de ressentir un bien-être. En raison, notamment des troubles du comportement des patients atteints de cette maladie, et des capacités de dialogue altérées, les soins bucco-dentaires sont particulièrement difficiles, surtout quant à l’évaluation de la douleur dentaire. Les difficultés des soins sont aggravées par le fait que le patient est de moins en moins capable de prendre soin de lui-même, notamment en ce qui concerne les techniques d’hygiène bucco-dentaire. De plus, les traitements médicamenteux anti-Alzheimer entraTnent des manifestations buccales qui nécessitent aussi un suivi bucco-dentaire. Cette prise en charge requiert donc une approche spécifique et ce type de patients nécessitent des soins adaptés. Dans tous les cas, le challenge du praticien sera aussi d’identifier le stade de la maladie et de stabiliser l’état bucco-dentaire du patient au stade précoce, avant que l’aptitude à coopérer de celui-ci ne se dégrade. L’objet de cette thèse, qui est une revue de littérature, est de présenter la stratégie globale de soins que peut adopter le chirurgien-dentiste en 2013, afin de prodiguer un traitement adapté à chaque patient atteint de la maladie d’Alzheimer.
