Introduction : Contrairement à la métropole, où il est plus faiblement pratiqué, le tiers payant est généralisé pour les consultations de médecine générale, à La Réunion, depuis 2002. L'objectif de cette étude était d'explorer les représentations du tiers payant généralisé par les médecins généralistes libéraux de La Réunion. Méthode : Une étude qualitative par entretiens semi-dirigés individuels et focus group avec analyse thématique a été réalisée. Résultats : Quatorze médecins généralistes ont été interrogés du 18 juin au 31 aout 2020. Ils ont des représentations hétérogènes du tiers payant généralisé. Certains en semblent satisfaits, évoquant un système efficace, améliorant l'accès aux soins des patients, désengorgeant les urgences et permettant de séparer la consultation du paiement de celle-ci. A contrario, le tiers payant peut aussi être associé à une augmentation des couts et des charges administratives des médecins, à l'inflation du nombre de consultations, à la dévalorisation de la médecine et à une dépendance accrue aux organismes payeurs. Pour remédier à ces problématiques, ils proposent l'instauration d'un payeur unique ou d'aides financières et l'optimisation de l'efficience et de la communication des organismes complémentaires. Ils prônent l'information, l'éducation et le contrôle des patients et des médecins concernant le cout de la santé. Discussion : La diversité des représentations retrouvée à La Réunion est similaire à celle des médecins métropolitains. Une étude globale auprès des patients, des médecins, de la CGSS et des organismes complémentaires permettrait de faire un état des lieux des 18 années de pratique du tiers payant généralisé à La Réunion.

Introduction : Le projet de généralisation du tiers du payant intégral a suscité une forte opposition des médecins généralistes métropolitains. Or, ce système est en place sur l'île de la Réunion depuis 2002. L'objectif principal de cette étude était d'explorer les représentations des médecins généralistes réunionnais sur la pratique du tiers payant intégral et généralisé. Méthode : Etude qualitative par entretiens individuels semi-directifs auprès de médecins généralistes réunionnais avec analyse par théorisation ancrée. Résultats : Quatorze entretiens ont été réalisés. Le tiers payant intégral et généralisé était pratiqué majoritairement par les médecins. Le tiers payant sur la part obligatoire était efficient mais les dysfonctionnements des organismes complémentaires étaient responsables de nombreux impayés. Ce dispositif générait une surcharge administrative conséquente et des frais logistiques importants pour les praticiens. Le secrétariat tenait une place centrale dans la gestion de ce système. Si le tiers payant favorisait l'accès aux soins pour la population, il permettait une dérive des comportements et une déresponsabilisation globale des acteurs du système de santé, induisant une augmentation des actes médicaux. Les médecins étaient en faveur d'une généralisation nationale du tiers payant intégral mais estimaient que ce dispositif ne pourrait être déployé sans amélioration préalable du système actuel, au risque de compromettre sa pérennité. Conclusion : Ces résultats ont permis de mettre en perspective les craintes des médecins généralistes métropolitains et de proposer des pistes de réflexion sur l'amélioration de ce dispositif afin d'en garantir l'efficience.

Introduction : En médecine générale, la pratique du tiers payant tend à se généraliser en France métropolitaine. Les médecins généralistes réunionnais ont la particularité de réaliser un tiers payant intégral à l'ensemble de la population depuis 2002. L'objectif de cette étude était d'explorer les représentations des patients réunionnais concernant cet usage. Méthode : Etude qualitative par entretiens individuels semi-directifs auprès de patients réunionnais avec analyse par théorisation ancrée. Résultats : Vingt-et-un entretiens individuels ont été réalisés du 15 mai au 17 septembre 2019 dans 7 salles d'attente de cabinets de médecine générale de la Réunion. Les patients interrogés étaient satisfaits de la pratique du tiers payant intégral, qu'ils qualifiaient de juste et d'équitable. L'accès aux soins était largement favorisé par ce système. La relation médecin-patient était de qualité et affranchie du rapport financier. Sa généralisation sur le plan national était vue comme une réforme souhaitable. Les connaissances des patients interrogés sur le système de santé étaient faibles. La plupart ne connaissait pas le prix d'une consultation chez le médecin généraliste. L'habitude de ne pas payer était profondément ancrée. La gratuité apparente et la réticence à payer pour sa santé entrainaient une certaine déresponsabilisation des patients vis-à-vis du système de santé. Conclusion : Le tiers payant intégral favorise l'égalité dans l'accès aux soins pour la population générale. Cependant, il est possible à long terme que cette pratique induise une certaine forme de déresponsabilisation des patients qu'il conviendrait de prévenir en cas de généralisation sur le plan national.

Originally published in 1976 and on the basis of extended case histories, Eysenck showed how experts dealt with problems which arose in the course of behaviour therapy. It showed how they formulated hypotheses about causation and treatment, and used these to structure the methods employed; and how they changed their hypotheses when treatment showed them to have been mistaken. The prime aim was to demonstrate the complexities involved in even apparently simple cases, and the need to base treatment on a proper understanding of the dynamics of the case.
