La prothèse fixée sur implants pose souvent la problématique du volume osseux disponible. La perte des dents entraîne la résorption de leur support, l'os alvéolaire. Ce processus est progressif et irréversible : un édentement ancien, non restauré, est associé à une résorption avancée qui modifie l'anatomie des arcades maxillaire et mandibulaire. La proximité avec les éléments anatomiques à risque peut alors compromettre la mise en place d'implants dans les secteurs postérieurs. Afin d'assurer un support suffisant à la réhabilitation prothétique, des techniques d'aménagement chirurgical ont été développées dans le but de restaurer un volume osseux compatible avec la chirurgie implantaire. Ces interventions, qui requièrent une formation spécifique du chirurgien-dentiste et un plateau technique approprié, peuvent être à l'origine de complications per ou postopératoires. Elles imposent de fait une chirurgie et un coût supplémentaires pour le patient mais également le respect d'un délai de cicatrisation avant la mise en charge de la prothèse. Le développement de la CFAO et la sophistication des nouvelles pièces de connexion supra-implantaires offrent aujourd'hui une simplification des protocoles chirurgicaux et prothétiques : des techniques adaptatives permettent la mise en charge immédiate de prothèses complètes fixes sur un nombre réduit d'implants, placés dans les secteurs antérieurs en contournant les obstacles anatomiques.

Les progrès en implantologie et la fiabilité des prothèses fixées implanto-portées sur le long terme permettent de répondre aux attentes du patient non seulement sur un plan esthétique mais également fonctionnel. La réussite d'une réhabilitation implanto-portée globale passera par la validation d'une phase d'analyse pré-implantaire d'un point de vue clinique, anatomique, prothétique et radiologique. Dans certains cas lors, de déficit osseux trop important pour pouvoir envisager la pose d'implant de manière optimale, il sera nécessaire de réaliser une greffe osseuse pour restaurer le volume osseux. Ensuite, le choix du nombre d'implants par arcade dentaire (4, 6, 8 ou encore 10 implants) dépendra principalement du volume osseux disponible mais également du projet prothétique. The European Register of Cleft Lip and Palate Services will be produced as a result of the concerted action project: Standards of Care for Cleft Lip and Palate in Europe: EUROCLEFT funded by the European Commision under its BIOMED II and INCO COPERNICUS programmes. The project, aimed at developing a network of cleft care teams across Europe, ran for three years between 1996 and 1999 and aimed broadly to improve the effectiveness and efficiency of care for European children with clefts of the lip and/or palate. Clefts of the lip and/or palate are birth defects affecting 1:500 live births. Successful rehabilitation requires a complex multidisciplinary mix of services.
