L'objectif de cette étude était de déterminer si, au cours d'une hospitalisation non programmée en court séjour gériatrique, il y avait ou non une modification quantitative et/ou qualitative des prescriptions existantes. Cette étude a été réalisée chez 494 patients. Pour chacun d'entre eux, nous avons rempli un tableau indiquant : les médicaments habituellement prescrits aux patients avant leur séjour hospitaliers ; Les médicaments sortis à leur sortie et donc les motifs de modification s'il y en avait. Les médicaments étaient répertoriés en fonction de leur classe thérapeutique. Notre étude a révélé une faible variation quantitative des prescriptions : 5,8 médicaments par patient à l'admission, contre 5,7 à la sortie. En revanche, sur le plan qualitatif, 68 % des médicaments antalgiques ont été modifiés, 42% en Hépato-gastro-entérologie, 40 % en neuro-psychiatrie, 34 % des médicaments du système métabolisme- nutrition-endocrinologie et 32 % des médicaments du système cardio-vasculaire. Parmi les motifs de modification les plus fréquents, on retrouvait les motifs " Nouvelle Pathologie Diagnostiquée " (32 %), " Absence Actuelle d'Indication " (26,6%) ; et " Optimisation Thérapeutique "(17.1%). Les motifs liés à la iatrogénie constatée ou potentielle étaient moindres : Respectivement 6,3% et 3,5%. Ils étaient principalement en rapport avec les médicaments à visée cardio-vasculaires et neuro-psychiatriques. Ils occupaient respectivement 69 et 29 % de la iatrogénie constatée. Les conclusions de notre étude corroborent les travaux antérieurs concernant la surconsommation médicamenteuse chez les sujets âgés. Si les causes de cette sur-consommation sont multiples, les conséquences le sont autant et peuvent parfois être très graves. Aussi, les prescripteurs ne doivent pas hésiter à" dé-prescrire " ou à modifier les prescriptions de prise en charge thérapeutique.
