Introduction : la prise en charge de malades atteints de coronavirus disease 2019 (Covid-19) en structure des urgences (SU) nécessite d’évaluer précisément les critères de gravité, en particulier en période épidémique. La Covid-19 est une maladie évolutive et l’aggravation respiratoire est un élément pronostic important. Notre objectif était de décrire le profil évolutif clinique d’une population française atteinte de Covid-19 et de déterminer s’il était un facteur de mortalité. Matériel et méthodes : étude observationnelle rétrospective menée aux Hôpitaux Universitaires de Strasbourg incluant les patients hospitalisés depuis la SU en 2020 pour une infection au severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2). Elle évaluait les délais entre le début des symptômes (J0) et l’initiation d’une oxygénothérapie, et entre le J0 et l’apparition des complications hospitalières. Par analyse multivariée, nous avons recherché les facteurs associés à l’oxygénothérapie précoce et à la mortalité intra-hospitalière. Résultats : l’analyse concernait 699 patients. Par rapport au J0, le jour moyen d’initiation d’une oxygénothérapie était le 7ème jour ± 4, et d’une ventilation mécanique le 9ème jour ± 4. L’initiation précoce d’oxygénothérapie (avant le 6ème jour) était indépendamment associée au sexe féminin et à la mortalité intra-hospitalière. L’absence d’HTA était un facteur protecteur d’oxygénothérapie précoce. En analyse multivariée, la mortalité intra-hospitalière était associée à un âge ≥ 75 ans, à une cardiopathie ischémique, et à une immunosuppression. Le jour moyen de mortalité intra-hospitalière était le 16ème ± 14. Conclusion : le jour moyen d’initiation d’oxygénothérapie est le 7ème jour ± 4. Une oxygénothérapie précoce, un âge ≥ 75 ans, une cardiopathie ischémique et une immunosuppression sont associés à une forme grave de Covid-19. Ces éléments pourraient être réunis dans un score afin de repérer rapidement, dès l’admission en SU ou en consultation de médecine générale, les patients nécessitant surveillance accrue et hospitalisation.
