Introduction : le choc hémorragique est un choc hypovolémique secondaire à une perte importante du volume sanguin. Le décès est rapide en l'absence de contrôle de l'hémorragie. Pour cela des recommandations formalisées d'experts éditées en 2014 par la société française d'anesthésie et de réanimation (SFAR) détaillent la prise en charge en aigu du patient en choc hémorragique. Elles dictent des mesures spécifiques de réanimation hémostatique, notamment en termes de transfusion. Nous avons donc décidé d'évaluer nos pratiques de prise en charge du choc hémorragique et de les comparer au gold standard dicté par la SFAR. Méthodes : cette étude rétrospective, observationnelle et monocentrique a été réalisée sur le Centre hospitalier Universitaire (CHU) de Toulouse. Du 01/09/2018 au 01/06/2021, 246 dossiers de patients identifiés comme étant en choc hémorragique à leur admission aux urgences ont été analysés. Le critère de jugement principal était le ratio PFC:CGR dont avaient bénéficié les patients transfusés au SU. Résultats : l'étiologie du choc était médicale pour 204 patients, traumatique pour 42. Seulement 20 patients, soit 9% de l'effectif a reçu un ratio transfusionnel adapté aux recommandations (au minimum 1:2). L'expansion volémique a été bien conduite pour au moins 75% des patients. La nature du soluté répondait aux recommandations pour 100% des patients. Nous avons retrouvé un respect des recommandations dans la moitié des cas pour le monitoring du fibrinogène et moins de 10% pour sa substitution. La recommandation de maintenir une calcémie ionisée dans la norme a été prise en compte dans 13,5% des cas. L'utilisation d'amines n'a été réalisée, conformément aux recommandations, que dans 15% des cas, tout comme l'utilisation de l'acide tranexamique ainsi que la transfusion de plaquettes pour un taux
