Since 2008, more than 289 million insecticide-treated mosquito nets have been delivered to sub-Saharan Africa, enough to protect three quarters of the 765 million persons at risk of the disease. Over the last decade, 11 countries in the Africa Region and 32 countries in other regions showed reductions of 50% or greater in either confirmed malaria cases or malaria admissions and deaths. Malaria control is making a major contribution to reducing mortality in children less than 5 years old and to attaining the health-related Millennium Development Goals.

CONTEXTE : La carie des dents temporaires non traitée est la maladie non transmissible infantile chronique la plus courante dans le monde et en France. Les inégalités territoriales d'accès aux soins dentaires, les disparités de moyens pour y accéder ainsi que le coût important pour la société font de la carie dentaire de l'enfant un vrai enjeu de santé publique. Pourtant, des moyens de prévention existent, notamment le recours au dentifrice fluoré. L'objectif de cette étude est de synthétiser les preuves sur l'impact du fluor topique par dentifrice dans la prévention primaire de la carie dentaire chez les enfants de moins de 6 ans. METHODE : Revue parapluie portant sur les enfants de moins 6 ans, exposés au dentifrice fluoré vs non exposés au fluor par dentifrice, avec pour critère de jugement l'augmentation moyenne du dmfs/ DMFS ou dmft/DMFT (PICO). RESULTATS : 4 revues systématiques ont été incluses sur les 910 identifiées. Il existe une différence statistiquement significative en faveur de l'utilisation des dentifrices fluorés de plus de 1000ppm, pour les 3 revues ayant effectué des méta-analyses, les comparant à un placebo ou l'absence d'intervention : PF groupé 30% IC à 95% (18 à 43%), DMS -0,24 IC à 95% (-0,36 à -0,13%), DM -1,86 IC à 95% (-2,52 à -1,21%). Bien que plus efficace que l'absence d'intervention ou un placebo, aucun des résultats portant sur les dentifrices de moins de 1000ppm de fluor n'est significatif. CONCLUSION : Le dentifrice au fluor est efficace chez les enfants de moins de 6 ans en prévention primaire de la carie dentaire. Une relation dose-réponse semble exister dans l'efficacité du dentifrice au fluor. Des études complémentaires sont nécessaires pour affirmer ce propos. Le médecin généraliste devrait être partie prenante de la prévention de la carie dentaire précoce par une information fiable et systématique auprès des parents et de leurs enfants, lors du suivi habituel pédiatrique.
