La déshydratation est une cause fréquente d'hospitalisation dans une population de personnes âgées en constante augmentation. La prise en charge de cette problématique par le médecin généraliste permet d'éviter des hospitalisations. L'objectif de notre travail était de déterminer la place de l'hydratation artificielle de la personne âgée vivant à domicile ou en EHPAD dans la pratique du médecin généraliste. Une revue systématique de la littérature, incluant des articles publiés entre janvier 2000 et novembre 2016, a été réalisée selon les recommandations PRISMA. Pubmed, EM Premium, Banque de données en Santé Public, SUDOC, Google Scholar ont été explorés. Des grilles d'évaluation ont été utilisées pour apprécier la qualité des articles. Chaque étape a été réalisée indépendamment par deux chercheurs. Sur 7090 références retrouvées, 11 ont été inclues. L'hypodermoclyse est la voie d'administration la plus utilisée pour l'hydratation artificielle de la personne âgée à domicile du fait de son efficacité, de son innocuité et de sa facilité d'emploi. Les effets indésirables sont rares, le plus souvent locaux et spontanément réversibles. Ses indications sont le traitement de la déshydratation modérée, sa prévention et les soins de fin de vie. Les modalités d'administration sont hétérogènes selon les médecins. L'hydratation a une valeur symbolique. Les médecins sont influencés par leurs représentations, leurs connaissances, leur expérience mais aussi par le patient et sa famille. En contexte de fin de vie, la prescription d'une hydratation artificielle est controversée. Son arrêt peut être bénéfique, l'inconfort du patient n'étant pas majoré après cette décision.

Childen's palliative care has developed rapidly as a discipline, as health care professionals recognize that the principles of adult palliative care may not always be applicable to children at the end of life. The unique needs of dying children are particularly evident across Africa, where the scale of the problem is overwhelming, and the figures so enormous that they are barely comprehensible: over 400,000 children in Africa died from AIDS in 2003, and out of the 166,000 children a year diagnosed with cancer, 85% of these are in the developing world. Despite the enormous need, provision of children's palliative care in Africa is almost non-existent, with very few health workers trained and confident to provide care for dying children. The challenges of providing palliative care in this setting are different to those in more developed countries, contending with the shortage of physical and human resources in addition to the vast scope of the care needed.
