Pour ce travail de thèse, la question de recherche est : quel est l'intérêt pédagogique d'une journée de consultation par semaine pour le futur médecin généraliste en SASPAS. Méthode: Étude d'opinion quantitative par groupes d'intérêt, interrogés avec des questionnaires type échelle de Likert. Les groupes étant: les internes de phase de consolidation, les internes ayant effectué un stage avec une journée de consultation par semaine en médecine interne lors de leur SASPAS, les médecins généralistes adressant des patients à la consultation de médecine interne et le DMG. Le maître de stage de médecine spécialisé a été interviewé pour recueillir son avis. Conclusions : Selon les intervenants sondés, cette forme de SASPAS présente un intérêt pédagogique dans la formation de l'interne en médecine générale. Elle paraît profitable pour le développement des compétences de l'interne et du futur généraliste en exercice. De plus, les internes seraient enclins à réaliser ce type de stage. Elle présente néanmoins un frein quant à sa mise en œuvre et à l'organisation dans les secteurs de spécialité

Introduction : II existe un décalage entre la vision qu'ont les jeunes médecins de leur métier et les conditions réelles de celui-ci. Afin de les aider à choisir leur mode d'exercice, cette étude recherche les intérêts de chaque organisation d'agenda de consultations pratiquée par les médecins de premier recours français. Méthode : Une étude qualitative par entretiens semi-structures en variation maximale a été conduite auprès de médecins généralistes installés. Il s'agissait de les questionner sur leurs motivations à choisir, à conserver et à recommander leur pratique. Résultats : Le mode de fonctionnement du cabinet dépend de l'organisation d'agenda choisie qui détermine l'accessibilité, la charge et le rythme de travail, la préparation et la durée des consultations, le temps d'attente du patient. Lors du choix de l'organisation de son agenda de consultations, le médecin doit tenir compte non seulement de l'offre locale de soins préexistante mais également des décisions d'exercer ou non en groupe, d'avoir ou non un secrétariat, de créer ou de reprendre une activité. Les pratiques Sans Rendez-Vous et « Surbooking » favorisent l'accessibilité. Les pratiques Sur Rendez-Vous et en Carve Out favorisent la durée des consultations. Le Mixte et le Système Mixte en Groupe, en combinant différents systèmes, allient les avantages et limitent les inconvénients de chacun. Discussion : Le choix de l'organisation de l'agenda dépend des objectifs du médecin en termes de conscience professionnelle, de capacité à satisfaire la demande du patient, de considération de sa santé, et de respect de ses temps de vie privée. Cette étude propose donc les questions préalables qu'un médecin doit se poser afin de préciser ses objectifs et d'être satisfait ultérieurement de son choix. Conclusion : Cette étude présente les avantages et les inconvénients de chaque organisation et propose au médecin les réflexions utiles permettant de faire un choix éclairé concernant son agenda de consultations.
