Patient-centered care (PCC) is a concept characterized by the clinician educating and communicating with the patient in a compassionate manner. The clinician and patient share responsibility in the decision-making process, advocate for injury prevention, and promote a healthy lifestyle. PCC is a component of evidence-based practice (EBP), which has received national attention in the profession of athletic training. PCC is also a recently added core competency to the post-professional athletic training education programs. Objective: To (1) examine program directors' perceptions of new standards; (2) describe the methods in which the program directors emphasize PCC; and (3) explore how the students are learning these concepts. Design: Cross-sectional survey. Setting: Self-administered online survey. Participants: Sixteen Commission on Accreditation of Athletic Training Education (CAATE) post-professional athletic training education program directors and the students currently enrolled. Intervention(s): A previously validated survey was utilized to assess usage and opinion of patient-rated outcome measures (PROMs) with questions added to assess the opinion of PCC. Open-ended questions were also added to assess student understanding of the definition of PCC and PROMs as well as how the program directors are implementing the changes. The survey was entered in an online survey database and the program directors were sent an email containing a link to a specific survey. Included in the email was a request that the directors send a list of email addresses for their students. The students were sent a separate link to a specific survey. Main Outcome Measures: The program directors perceptions on the new changes, how they are implementing the changes, students' knowledge on PCC and PROMs, and students are currently implementing this concept. Results: All of the program directors welcomed the curriculum change of emphasizing PCC and stated that they teach their students about PROMs in the classroom. Five themes emerged from the students' answers including: Individualized care centered on the patient's needs, values, goals, and concerns (60.5%); involving the patient in the treatment plan (23.3%); holistic care (16.3%); utilizing EBP and/or PROMs (18.6%); and communicating with and educating patients fully (9.3%). When students were asked about the definition of PROMs, there were four themes that emerged from their answers including: how the patient feels the injury impacts their lives in terms of their health-related quality of life and function in daily life (45.2%); how the patient feels their treatment/intervention is going (33.3%); questionnaire/survey completed by the patient (26.2%); and gives the clinician insight to the patients' perspective (16.7%). Conclusion: Students have a limited understanding of the true definitions of PCC and PROMs. Young clinicians need to be educated on the concept of PCC and how using PROMs apply to PCC in order to enhance EBP in clinical athletic training.

"A person-centered model of healthcare is considered the gold standard of care; however, athletic training continues to operate within a biomedical model of care. More research is needed to understand how to best incorporate person-centered care into the culture of care within athletic training. This study evaluated the effectiveness of a four-part learning module on athletic training students' understanding, use and perceived use of person-centered care in athletic training. The learning module was incorporated into an existing course on psychosocial aspects of healthcare. Ten students (9 female, 1 male) participated in the study. Study measures included a clinical evaluation observation measuring participants' use of person-centered care in a clinical setting and a pre-post survey measuring participants' perceived use of person-centered care in a clinical setting. Results of the paired t-tests on the clinical evaluation observation and pre-post survey items demonstrated a significant improvement in participants' perceived use of PCC in their clinical practice. Findings indicate that actively incorporating person-centered care into the curriculum of athletic training education can have a significant positive impact on students' use and perceived use of person-centered care in their clinical practice. More research is needed to explore other educational methods for incorporating person-centered care across the athletic training educational curriculum as well as the impact of a person-centered culture of care on patients and clinical outcomes in athletic training settings."--Abstract from author supplied metadata.

The Dissertation of Clinical Practice Improvement highlights the major constructs of the Doctorate of Athletic Training (DAT) program, including the need for scholarship that leads to professional growth and a focus of action research that displays clinical outcomes of professional practice. My scholarship and professional growth began with the creation of a Plan of Advanced Practice (PoAP), outlined in Chapter 2 that captured my strengths, weaknesses, and goals (along with accomplishments) for advancing my clinical professional practice within the DAT and for the next 5--10 years of professional practice. Through the PoAP, I reflected on the desire to improve my patient care through the utilization of new interventions and paradigms while reflecting upon the need to improve my clinical setting (the secondary school setting) to provide quality patient care. In Chapter 3, I present clinical patient-oriented outcomes while within the DAT program that focused on learning and implementing new interventions into my own clinical practice. Chapter 4 is a literature review that synthesizes the current and previous research related to athletic trainers in the secondary school setting and research relating to the study and improvement of quality health care in other medical fields. Once the problems and needs for improvement in the secondary school setting were addressed, I focused the majority of my research project, Chapter 5, on performing a quantitative and qualitative assessment of the secondary school setting from a third-party stakeholder's perception of the athletic training services. The research study was through an action research process that deals with real problems and real-life situations by evaluating the local effectiveness of professional practice based on the interpreter's individual paradigm, and then the practitioner can create an outstanding solution to a local issue that can be used everywhere. After performing the research study, I was able to determine the focus of stakeholders in the secondary school setting in regards to the athletic trainer's time spent on duties related to patient care.
