The increasing prevalence of preterm birth in the United States is a complex public health problem that requires multifaceted solutions. Preterm birth is a cluster of problems with a set of overlapping factors of influence. Its causes may include individual-level behavioral and psychosocial factors, sociodemographic and neighborhood characteristics, environmental exposure, medical conditions, infertility treatments, and biological factors. Many of these factors co-occur, particularly in those who are socioeconomically disadvantaged or who are members of racial and ethnic minority groups. While advances in perinatal and neonatal care have improved survival for preterm infants, those infants who do survive have a greater risk than infants born at term for developmental disabilities, health problems, and poor growth. The birth of a preterm infant can also bring considerable emotional and economic costs to families and have implications for public-sector services, such as health insurance, educational, and other social support systems. Preterm Birth assesses the problem with respect to both its causes and outcomes. This book addresses the need for research involving clinical, basic, behavioral, and social science disciplines. By defining and addressing the health and economic consequences of premature birth, this book will be of particular interest to health care professionals, public health officials, policy makers, professional associations and clinical, basic, behavioral, and social science researchers.

Introduction : Le canal artériel persistant (CAP) chez l'extrême prématuré, son traitement et les conséquences à court et moyen terme, ont été largement étudiés mais restent controversés. Nous avons évalué leur morbimortalité néonatale et leur développement neurologique selon la présence d'un CAP et sa stratégie thérapeutique. Matériels et méthodes : Dans cette étude prospective, ont été inclus tous les prématurés nés entre 23 et 26 semaines d'âge gestationnel (AG) en 2010 et pris en charge en réanimation au CHU de Bordeaux. Le diagnostic et le traitement du CAP étaient réalisés selon le protocole du service. Nous avons comparé les caractéristiques de la population, la mortalité et les morbidités, selon la présence ou non d'un CAP, de son traitement curatif ou conservateur, médical ou chirurgical. Résultats : 70 patients d'un terme médian de 25 semaines d'AG [24 ; 25] et poids médian de 845g [751 ; 917] ont été inclus sur 12 mois. 82.9% présentaient un CAP, sans facteur de risque anténatal significatif. Le taux médian de dysplasie broncho-pulmonaire (DBP) sévère était significativement plus élevé chez les prématurés avec un CAP (20.7% versus 0%, p=0.0002) et qui ont subi une chirurgie (58.8% vs 9%, p=0.002), particulièrement quand celle-ci était précoce. Nous notions une tendance à plus de séquelles en présence d'un CAP (47% vs 28%, p=0.05) mais un risque plus faible de décès et/ou séquelles neurologiques en cas de chirurgie (24% vs 54%, p=0.05). Il n'y avait pas de différence significative entre les différents traitements concernant le taux d'entérocolite ulcéro-nécrosante (ECUN), d'hémorragie intra-ventriculaire (HIV), de rétinopathie oblitérante du prématuré (ROP), de mortalité et de séquelles globales. Conclusions : Chez l'extrême prématuré, la morbidité néonatale et les séquelles semblent augmenter en présence d'un CAP. La morbidité respiratoire augmente d e façon significative en cas de ligature chirurgicale, à l'inverse du risque de décès et/ou séquelles neurologiques. Ces résultats ne nous permettent pas de conclure sur un lien de cause à effet et seront à confronter à des études ultérieures.
