L'augmentation de l'espérance de vie entraîne une augmentation du vieillissement de la population qui provoque des pathologies multiples et des pertes d'autonomie responsables d'une fragilité particulière de ces patients nécessitant des prises en charge adaptées, notamment en milieu gériatrique. L'accueil des personnes âgées dans les hôpitaux se fait souvent par l'intermédiaire des services d'accueil des urgences; les modalités de prise en charge sont souvent mal adaptées du fait des délais d'attente, mais aussi du fait qu'il est difficile dans des services orientés d'abord vers la prise en charge des pathologies aiguës de réaliser une évaluation gériatrique. Il faut y ajouter les problèmes posés actuellement par l'insuffisance du nombre de lits d'accueil dans la filière gériatrique (court, moyen voire long séjour), tout ceci aboutissant à un nombre important de patients mal orientés. Ce travail de thèse a donc pour objectif d'évaluer les possibilités d'évaluation des patients âgés de 75 ans et + dans un service d'accueil des urgences à l'hôpital de Verdun, d'analyser les difficultés rencontrées et de voir en quoi une meilleure évaluation permettrait aussi une meilleure orientation. Elle permet aussi d'évaluer les besoins en lits de court séjour gériatrique pour accueillir dans les meilleures conditions les patients âgés fragiles qui justifieraient d'une telle structure. Ce travail s'organise en trois grandes parties : - La première partie porte sur des généralités: la fragilité en gérontologie, la pertinence de l'évaluation aux urgences par le score ISAR, la filière de soins gériatriques et enfin les données sur la Meuse et plus particulièrement sur les perspectives d'accueil des personnes âgées dans le projet d'établissement de l'hôpital de Verdun. - La deuxième partie traite du travail de recherche proprement dit et porte sur la discussion des objectifs, de la méthodologie et des résultats. Il repose sur le remplissage d'un questionnaire pour les patients de 75 ans et + accueillis aux urgences. Celui-ci a permis de préciser le profil des patients accueillis et de comparer leur orientation telle que proposée par le service d'urgence, leur orientation réelle et celle qui aurait été souhaitable d'un point de vue gériatrique (analyse sur dossier par un expert gériatre). Au total, ce sont 388 questionnaires qui ont été recueillis concernant 230 femmes et 158 hommes d'âge moyen 82,5 ans. Les résultats de ce questionnaire sont ensuite présentés. Il apparaît qu'il existe un certain nombre de divergences entre l'orientation réelle des patients et celle qui aurait été souhaitable. Ceci conforte l'intérêt qu'il y aurait à créer des lits de court séjour gériatrique dans cet établissement. - Ce travail se termine par une discussion relativement brève portant sur quelques données de la littérature et sur les perspectives d'évolution de la filière gériatrique à l'hôpital de Verdun.
