The life expectancy of the U. S. population is increasing. The advantages of fluoridated water, dental insurance, greater awareness of preventive oral healthcare measures, and more restorative dental services, have helped this generation of adults maintain their teeth into their advanced years. The demand for oral health care services will be greater and more complex that of previous generations. Challenges for the older adult, and the healthcare system providing for them, are complex. Increasing evidence of a link between oral health and overall health underscores the need to promote public health awareness of the need for dental care to be part of overall health care. Understanding the attitudes, expectations, intentions, and current level of oral and overall health knowledge, is vital to helping these individuals' transition into increasing levels of dependency with dignity and a high level of oral health related quality of life. This study used a qualitative approach to better understand these issues from the perspective of the older adult. A purposeful sample of 26 independent living adults age 65 and older in Northwest Arkansas was selected for this investigation. Semi structured interviews were recorded, transcribed, and analyzed using constant comparative thematic analysis typical of qualitative methods to code, categorize, and develop themes. Results revealed the emergence of five themes: 1) Difficulties accessing dental care, 2) Stoic independence, 3) Taking care of your mouth as part of overall health, 4) Relationships affecting oral health related quality of life, and 5) Supporting roles. Conclusions reveal a need to increase oral health literacy of the older adult population with attention to reducing the modifiable risk factors associated with dental diseases. Additionally, there is a need to reevaluate public policy related to providing dental services in both the private and public sector.

Abstract : Although the United States has experienced population improvements in oral health over the past 60 years, oral diseases remain among the most common chronic conditions across the lifespan. Moreover, low-income, racial/ethnic minority, and elderly members of the population experience barriers to oral health opportunities. These barriers result in a lack of oral health awareness and dental hygiene, fewer contacts with the oral health care system, and poorer oral health outcomes. This dissertation research integrates different data sources and methods toward an enhanced understanding of the barriers faced in accessing dental care among racial/ethnic minority older adults in an urban context, and identifying opportunities for reducing barriers to oral health and health care. Qualitative data were analyzed from focus groups with community-dwelling older adults who self-identify as African American, Dominican, or Puerto Rican to elicit the lived experiences of older adults in accessing dental care in their northern Manhattan neighborhoods. To do so, this dissertation leveraged an approach to systematically extricating and representing factors associated with the decision-making process in finding and visiting dentists. The qualitative data analysis revealed that older adults want more information about oral health and their dental care options, particularly affordable dental care in their neighborhoods. The results from this qualitative analysis support the need for community-based oral health promotion and care. To further contextualize these findings, spatial data and analysis techniques were used to assess information about older adult populations and community resources for oral health promotion activities and dental care in the study area. The analysis of older adults’ lived experiences is thus situated in the community context. Therefore, this dissertation contributes approaches that may serve to inform context-specific strategies to promote oral health equity.

Unlike previous generations, baby boomers are keeping their own natural teeth as they enter into older age. There is a growing body of research demonstrating the link between oral diseases and common chronic diseases. Therefore, it is imperative that older adults possess the necessary skills and knowledge, and participate in preventive oral health behaviours to remain healthy. The purpose of this exploratory, mixed-methods online study was to explore the influence of oral health literacy (OHL) and oral health chronic disease knowledge (OHCDK) on oral care behaviours (OCBs). A convenience sample of 69 community dwelling men (n=19) and women (n=50) was recruited to participate in the Oral Health Chronic Disease Knowledge Study. This Internet based study utilized 4 separate questionnaires: The Oral Health Literacy-Adult Questionnaire, The Oral Health Chronic Disease Knowledge Questionnaire, The Background Information Questionnaire, and The Oral Care Behaviours Survey. Findings from The Oral Care Behaviours Survey were further categorized to create the dependent variable: dental care behaviours, and the independent predictor variable: home dental aid use behaviour group. In addition, qualitative data was collected through the use of 9 open-ended questions. Participants ranged in age from 50 to 69 years, and were further subdivided into early boomers (n=34) with an average age of 63.9, and late boomers (n=35) with an average age of 54.4. All participants had achieved higher than high school education; over half of all participants (52 %) reported living in an urban setting, and 46 % had an annual household income of greater than $75,000. All participants had some natural teeth and 72 % had access to private dental insurance. The mean OHL score for the total sample was 13.36, 95% CI [12.62, 14.10] and the mean OHCDK score was 10.01, 95% CI [9.13, 10.89]. Based on a Pearson product moment correlation co-efficient procedure there was a low but positive correlation between total population OHL and OHCDK scores [r = 0.31, n =69, p = 0.008]. A dental care behaviours score was determined by adding together individual's total points related to the self-reported frequency of dental visits plus frequency of brushing and frequency of flossing. Scores could range from a maximum of 10 to a minimum score of 3, and higher scores denote less participation in positive dental care behaviours. The mean dental care behaviours score was 5.60, 95% CI [5.36, 5.84]. A backward step regression analysis determined the significant predictor model for dental care behaviours included: (1.46) dental insurance + (0.85) age cohort + (0.6) level of education.
