This book delineates that COVID-19 is a multisystem inflammatory disease and how its pathophysiology can predispose patients to an increased risk of neurological problems. Available data suggest the potential neuroinvasive capacity of the SARS-CoV-2 through direct viral damage and indirect entering the CNS by different routes including the vascular system, the olfactory and trigeminal nerves, the cerebrospinal fluid, and the lymphatic system. Furthermore, the pro-inflammatory cytokine storm and oxidative stress can induce microglial activation and damaging the blood-brain barrier, culminating in widespread neuroinflammatory processes. This acute neurotoxicity is clinically expressed as anosmia, ageusia, headache, nausea and vomiting. Other neurologic manifestations such as acute cerebrovascular diseases, encephalitis-based impaired consciousness, and meningitis are also described. The PNS can also be affected and clinical manifestations including Guillain-Barré syndrome, polyneuritis, Miller Fisher Syndrome, and other problems are described. A special issue concerns the neurocognitive dysfunction and altered consciousness manifested as delirium, agitation, and confusion. Non-specific symptoms such as dizziness, and seizures can accompany clinical pictures. Regardless of the admitting diagnosis, a high percentage of patients discharged from ICUs develop disabilities affecting physical, cognitive and psychological activities. The symptoms such as asthenia, memory disturbances, depression, sleep disturbances, anxiety, and Post-traumatic Stress Disorder (PTSD), can configure the so-called Post-intensive Care Syndrome (PICS). Multimodal management during the ICU stay and implementation of follow-up programs on patient discharge can reduce the incidence of this syndrome, improving the quality of life of surviving individuals. In this complex scenario, a careful clinical approach through reliable diagnostic tools, and epidemiological studies aimed at evaluating the dimensions of the problem also in economic terms, is urgently needed. This book represents a valuable aid for all healthcare professionals (intensivists, neurologist and psychiatrists, as well as others) involved in the management of these critically ill patients.

This book presents the variability of the effects of Covid-19 on the nervous system (NS), with the purpose to update content and images based on improved scientific evidence. Current available data show that involvement of the NS is frequent in patients with SARS-CoV-2 infection. The most common neurologic syndromes include cerebrovascular disorders, encephalopathies, inflammatory Central Nervous System (CNS) syndromes, peripheral neurologic disorders, psychiatric disorders. The pathophysiology of neurological manifestations is far from being understood. They can be coincidental, common complications of severe viral infection, or direct consequence of the viral infection either via indirect para-infective mechanisms or direct viral penetration of NS. Experimental animal models had previously demonstrated the neuroinvasive potential of SARS-CoV and the detection of viral particles in special structures such as the thalamus, nucleus ambiguous and nucleus of the solitary tract, suggesting that CNS invasion can contribute significantly to the severe outcome not only through direct damage to neurological structures, but also through a potential detrimental effect on cardiorespiratory responses. Up to now, the detection of SARS-CoV-2 RNA in the cerebrospinal fluid of COVID-19 patients has been reported occasionally and conclusive pathological demonstration of the virus in the CNS is lacking. In this scenario, the role of neuroimaging is fundamental. These considerations highlight the urgent need to better clarify the neurotropic potential of the SARS-CoV-2 virus, and to verify on human autoptic tissue the mechanisms demonstrated in the experimental animal model in order to develop potential strategies to prevent CNS invasion and to adapt treatment protocols based on neurological involvement. CT scan is useful to detect large hemorrhage and ischemic lesions, that have been reported in Covid-19 patients, but lacks identifying other possible neurological complications, such as microhemorrhage or encephalitis. MRI could overcome these limitations; in particular the use of specific sequences may reveal microvascular lesions that can occur during the disease course, according to the described pathogenesis. This book will be an invaluable tool for neuroradiologists, radiologists, neurologists, and all physicians involved in the pandemic.

Coronavirus disease 2019 (COVID-19), produces the severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), with atypical pneumonia. This infection is a global health challenge producing post-acute sequelae of SARS-CoV-2 infection (PASC), including neuro-PASC in which direct or indirect invasion of the virus into the brain causes immune dysregulation, hormonal disorders, elevated cytokine levels due to immune reaction and chronic inflammation, direct tissue injury, and persistent low-grade infection. The COVID-19 induced-neuroinflammation promotes blood-brain barrier (BBB) disruption, access of antigens and inflammatory factors into the brain, as well as infection or immune-mediated response in the neuromuscular system. It is therefore necessary to explore whether there is a close correlation with the adenosine pathway, the activation of the ACE receptor by the virus, the concentration of IL6 and the neuro-invasive effect of COVID-19. Other points of interest are the analysis of the autonomic dysfunction induced by cytokines; the effect of vaccine mRNA delivery in Central and Peripheral Nervous System, the long-term effects of Covid 19 infection and/or therapeutic approach in different pandemic countries.
