Objectifs : Notre objectif principal était d'identifier les motivations et obstacles au changement du comportement de prévention des médecins généralistes envers leur propre santé afin d'améliorer leur prise en charge médicale. Notre objectif secondaire était de proposer un outil simple, pouvant être utilisé au cours d'une consultation de prévention pour améliorer le comportement de prévention des médecins généralistes envers leur propre santé. Méthode : Nous avons effectué une enquête qualitative par entretiens semi-dirigés auprès de 20 médecins généralistes de la Vienne. La saturation a été atteinte au 17ème entretien. Le corpus a été analysé selon les méthodes de l'analyse thématique avec le logiciel N Vivo par deux chercheurs à partir des entretiens retranscrits par écrit. Résultats : Les motivations et les obstacles trouvés au comportement de prévention des médecins généralistes envers leur propre santé ont été classés en trois domaines : environnemental, professionnel et personnel. Pour en faciliter la perception et l'utilisation nous les avons présentés sous forme d'un tableau récapitulatif. Nous avons créé un outil simple, synthèse de nos résultats et de nos recherches bibliographiques menées notamment sur le modèle du changement de comportement de Proschaska et DiClemente et de la méthode reconnue de l'Entretien Motivationnel, visant à faciliter la réalisation d'une consultation dédiée au comportement de prévention d'un soignant et à la rendre plus efficace. Discussion : La discussion montre que dès le stade de la prévention, les médecins présentent une grande ambivalence dans la prise en charge de leur santé. De même les obstacles rencontrés dans le cadre de la pathologie sont fréquemment déjà présents au stade de la prévention. Sur le plan des motivations comme sur celui des freins, l'influence du statut de médecin est très importante. Toutefois celles de sa personnalité et de son environnement matériel et affectif ne sont pas à négliger et tendent à nous rappeler que le médecin est avant tout une personne qui nécessite, au même titre que le patient lambda, une prise en charge dans sa globalité. Conclusion : En matière de prévention, la prise en charge d'un soignant nécessite subtilité et rigueur. L'écoute empathique et l'absence de jugement sont indispensables pour accepter l'ambivalence et rouler avec les résistances de cette population particulière. Une approche globale selon le modèle biopsychosocial de Engel facilite l'expression des obstacles et motivations au changement de comportement. Le support à la consultation de prévention d'un soignant que nous proposons pour améliorer la prise en charge du comportement de prévention des médecins généralistes envers leur propre santé doit maintenant être testé.
