Face au vieillissement de la population et à l'accroissement des admissions des personnes âgées dans les services d'urgences, nous nous sommes intéressés au recours spécifique des personnes âgées institutionnalisées. Nous avons voulu connaître les motifs et les circonstances de recours aux urgences de ces patients âgés, poly pathologiques, dépendants et établir un état des lieux de la prise en charge et de l'orientation de ces patients à partir du SAU du CHU de Clermont-Ferrand. Il s'agit d'une part, d'une étude prospective descriptive portant sur 137 recours observés sur la période de juin à décembre 2005 et, d'autre part, d'une étude rétrospective avec évaluation qualitative de la prise en charge au SAU. Les personnes âgées institutionnalisées ayant recours au SAU sont âgés de 85,7 ans en moyenne avec une prédominance féminine et proviennent très majoritairement (84%) des Etablissements Hébergeant des Personnes Agées Dépendantes (EHPAD). Ils reviennent souvent au SAU. Ils sont le plus souvent adressés par leur médecin traitant et non accompagnés. Le recours médical est prépondérant (55%). Les recours aigus (65%) réalisés dans les 24 heures qui suivent l'apparition des symptômes sont majoritairement chirurgicaux et traumatiques. Les symptômes évoluant depuis plus de 24 heures représentent 35% des recours et sont majoritairement médicaux. Il n'y a pas, le plus souvent, programmation de cette hospitalisation ni investigations diagnostiques. L'hospitalisation est l'orientation la plus fréquente et concerne 69% des patients. Les sujets âgés présentant une pathologie précise relevant d'un service de spécialité sont le plus souvent hospitalisés dans ce service. Pour des symptômes multiples ou frustes, les patients sont accueillis par des services de médecine générale (post-urgences, gérontologique). Enfin, les moyens diagnostiques et thérapeutiques mis en oeuvre aux urgences sont insuffisants eu égard au diagnostic ou à l'hypothèse diagnostique émis. Le SAU est devenu une porte d'entrée pour ces patients âgés, un accès préalable à une hospitalisation dans un autre service. L'hôpital doit s'adapter à la réalité d'une nécessité d'hospitalisation pour ces patients âgés et doit pouvoir leur offrir une possibilité de prise en charge de proximité dans des lits de médecine polyvalente. L'amélioration de la prise en charge de ces patients gériatiques passe, en outre, par une préparation de l'hospitalisation en amont au sein des EHPAD et par une collaboration avec les services hospitaliers. Enfin, une sensibilisation des médecins urgentistes aux spécificités de la prise en charge des sujets âgés est souhaitable.

Face au vieillissement de la population et à l’accroissement des admissions de personnes âgées dans les Services d’Urgence, nous avons souhaité faire un état des lieux au SAU du CHU de Clermont-Ferrand. Notre objectif était d’étudier la qualité de la prise en charge aux urgences des personnes de plus de 70 ans en analysant leur devenir à un mois et trois mois (ré-hospitalisations, décès, institutionnalisations) après leur admission au SAU. Le critère de jugement portait sur le bien fondé de l’orientation initiale (sortie des Urgences, hospitalisation) adaptée au niveau de fragilité. Afin d’évaluer cette orientation, nous avons choisi comme référentiel de tri, la grille SEGA. Il s’agit d’une étude prospective, avec un suivi de trois mois, menée au SAU du CHU de Clermont-Ferrand. Notre cohorte comprenait 346 patients âgés de plus de 70 ans, entrés au SAU entre novembre 2005 et janvier 2006. Le recueil a pris en compte les données démographiques de la population étudiée, les différents marqueurs de fragilités entrant dans la grille SEGA, le motif d’hospitalisation initial, l’orientation en sortie du SAU, puis le devenir à un mois et trois mois de l’admission : retour à domicile et modification des conditions de vie, ré-hospitalisation, décès, institutionnalisation. Le jour de l’entrée, les 346 patients inclus ont été répartis après calcul du score en 3 groupes SEGA : groupe « Peu Gériatriques », 157 malades ; groupe « Moyennement Gériatriques », 74 malades et groupe « Très Gériatriques », 115 malades. Nous retrouvons un taux d’hospitalisation de 75% le jour de l’entrée. Un mois après, 25 (7,23%) patients ont été ré-hospitalisés pour la même cause, 9 (2,83%) ont été institutionnalisés et 34 (9,83%) sont décédés. Entre J1 et J90, 51 (14,74%) patients ont été ré-hospitalisés pour la même cause, 18 (5,20%) ont été institutionnalisés et 59 (17,05%) sont décédés. L’analyse statistique fait apparaître que l’appartenance à l’un des trois groupes SEGA et la valeur absolue du score SEGA ne sont pas prédictives des échecs de décision des médecins (renvoi ou hospitalisation à tort). Au vu du travail réalisé, l’amélioration de la prise en charge des patients gériatriques passe autant par la bonne décision de tri aux urgences que par un dépistage plus répandu des patients âgés fragiles en amont de l’adressage aux urgences et relevant du champ d’action de la médecine générale.

History of Old Age is the first major study of the ways in which old age has been perceived in western culture throughout history. Georges Minois paints a vast fresco, starting with the first old man to relate his own story—an Egyptian scribe some 4500 years ago—and ending with the deaths of Elizabeth I and Henry IV in the sixteenth century.
