Introduction : Aujourd'hui, trois millions de personnes vivent avec ou après un cancer. Le Plan Cancer propose une prise en charge globale de la personne en tenant compte de l'ensemble de ses besoins afin de préserver la continuité et la qualité de vie des patients pendant et après la maladie. Le rôle des médecins généralistes semble donc fondamental dans la prise en charge des cancers. Dans cette perspective, nous avons cherché à identifier l'espace investi par les médecins généralistes dans le diagnostic des cancers. Notre question de recherche est donc : quelles sont les modalités de découverte des cancers dans un échantillon de patients suivis en médecine générale ? L'objectif principal de l'étude est d'analyser les modalités de découverte des cancers. Matériels et méthode : Nous avons mené une enquête épidémiologique observationnelle rétrospective. Les données ont été recueillies par auto-questionnaires anonymes envoyés à tous les médecins généralistes répertoriés dans la base de données d'une association nationale de FMC : FMC Action. Le questionnaire a été envoyé par mail aux 34 584 médecins figurant sur cette liste entre novembre 2016 et mars 2017. Nous avons étudié différentes variables : mode d'exercice des médecins, âge et sexe des patients. Nous souhaitions ensuite déterminer les modalités de découvertes des diagnostics de cancers : rôle du patient, diagnostic initié par le MG, par un tiers; part du dépistage de masse. Puis nous voulions savoir comment étaient confirmées ces suspicions diagnostiques. Résultats : D'après cette étude il apparait que : 80% des diagnostics sont initiés en soins primaires en collaboration avec le patient ; pour 65% des cas le médecin généraliste a permis la confirmation du diagnostic de cancer ; dans 65% des cas le MG est capable d'évoquer le diagnostic de cancer après avoir réalisé l'interrogatoire et l'examen clinique du patient ; dans 25% des cas le diagnostic a pu être suspecté sur la base des résultats d'un examen d'imagerie et dans 6% des cas d'après les résultats d'un bilan biologique. Dans notre étude : 2% des diagnostics de cancers ont été initiés par un confrère aux urgences, 11% des diagnostics par un confrère d'une autre spécialité en ambulatoire, 4% des diagnostics de cancers ont été initiés par un autre spécialiste en consultation par accès direct du patient, dans 67% des cas le médecin généraliste a initié la démarche de confirmation du diagnostic, dans 69% des cas le médecin a prescrit un examen d'imagerie, dans 8% des cas un examen biologique, dans 22% un examen cytologique permettant d'augmenter la forte probabilité d'un cancer. Conclusion : Il apparait donc bien que le médecin généraliste joue un rôle central dans la prise en charge initiale des cancers. Le tandem médecin généraliste-patient est extrêmement performant dans la réalisation des diagnostics de cancers. Cette étude démontre que l'approche d'une médecine centrée sur le patient est indispensable en cancérologie.
