Le nombre de femmes porteuses d'un utérus cicatriciel a augmenté ces dernières années, en corrélation avec l'augmentation du taux de césarienne. La TVBAC est aujourd'hui le plus souvent acceptée en cas de mise en travail spontané. Cependant, en cas d'indication médicale de déclenchement, le débat est tout autre. Nous observons alors un taux élevé de césariennes réalisées pour utérus cicatriciel et conditions locales défavorables. Méthodologie_ Nous avons réalisé une étude de dossiers bicentrique, comprenant 5 groupes de patientes toutes porteuses d'un utérus uni-cicatriciel (travail spontané, déclenchement par prostaglandines, par méthode mécanique, par oxytocine sur col favorable et défavorable). L'objectif est de définir les méthodes de déclenchement efficaces et sécuritaires pour la femme et le foetus/nouveau-né. Résultats et discussion_ Le déclenchement par oxytocine sur col favorable ne met pas en évidence de différence significative comparé à un travail spontané. Nous obtenons 60 % d'AVB avec l'utilisation de la méthode mécanique. Nous ne retrouvons aucune complication maternelle et une bonne tolérance foetale. Le déclenchement par prostaglandines provoque 2,32 % de rupture utérine et 20,93 % d'HPP. Le déclenchement par oxytocine sur col défavorable possède une efficacité égale à la méthode mécanique (63,3 % d'AVB) mais provoque plus de complications maternelles (6.66 % de rupture utérine) et une mauvaise tolérance foetale et néonatale. Conclusion_ Sur col favorable, le déclenchement par oxytocine n'augmente pas la morbidité materno-foetale et est donc une méthode efficace et sécuritaire. Le déclenchement par méthode mécanique semble être la meilleure alternative en cas de col défavorable.
