La revue de la littérature montre que les interventions en éducation thérapeutique (ETP) permettent d'accroître l'observance, de diminuer l'iatrogénie et d'améliorer la qualité de vie des patients atteints de maladies chroniques. En psychiatrie l'offre en ETP est particulièrement réduite (seulement 2% des programmes). Or chez les patients atteints de schizophrénie l'arrêt du traitement antipsychotique est la principale cause de rechute de la maladie. Il paraît donc primordial que des actions en ETP soient mises en place pour augmenter l'observance et ainsi diminuer la fréquence de survenue des rechutes. L'objectif de ce travail est de développer les différentes étapes ayant abouti à la réalisation d'un programme d'ETP élaboré par un groupe multidisciplinaire et intersectoriel. Il va permettre de familiariser le patient avec son traitement et ensuite favoriser son intégration dans le système de soins en ambulatoire. Celui-ci va devenir auteur de sa prise en charge.

Les comorbidités somatiques des patients atteints de schizophrénie posent un important problème éthique. Ces comorbidités péjorent le pronostic de la maladie, réduisent la qualité de vie et augmentent la mortalité précoce de ces patients. La mise en place de dispositifs spécifiques, accessibles et adaptés pour ces patients qui ne peuvent suivre un parcours de soins ordinaire, apparaît indispensable. Le Centre Hospitalier George Daumézon à Bouguenais (44) a pour projet de développer un programme d'éducation thérapeutique du patient autour de thématiques en rapport avec la santé physique : le programme « Corps et Santé ». Dans un contexte où des réticences sont apparues et se manifestent à l'encontre de ce dispositif venu déranger les habitudes de soin, ce programme ne fait pas consensus au sein de l'établissement. The good news is that systems' thinking is possible for anyone. It is always possible for an individual to understand feelings and to integrate them with their more rational brains. In so doing, an individual increases his or her ability to communicate despite misunderstandings or even rejection from important others. The effort involved in creating your Mindful Compass enables us to perceive the relationship system without experiencing it's threats. The four points on the Mindful Compass are: 1) Action for Self, 2) Resistance to Forward Progress, 3) Knowledge of Social Systems and the 4) The Ability to Stand Alone. Each gives us a view of the process one enters when making an effort to define a self and build an emotional backbone. It is not easy to find our way through the social jungle. The ability to know emotional systems well enough to take a position for self and to become more differentiated is part of the natural way humans cope with pressure. Now people can use available knowledge to build an emotional backbone, by thoughtfully altering their part in the relationship system. No one knows how far one can go by making an effort to be more of a self-defined individual in relationships to others. Through increasing emotional maturity, we can find greater individual freedom at the same time that we increase our ability to cooperate and to be close to others.
