Dans tout système de santé, les événements indésirables au cours des soins génèrent des coûts humains et financiers importants. Il n'existe encore que peu de travaux en France, particulièrement en soins primaires. La mise en place d'outils de collecte et d'analyse dédiés aux médecins généralistes, participe à une amélioration de la qualité des soins et à une diffusion de la culture de sécurité en ambulatoire. L'objectif principal de ce projet est de créer un système intégré de recueil et d'analyse d'événements indésirables utilisant internet, dédié aux médecins généralistes. Notre méthodologie a suivi quatre phases : Nous avons d'abord réalisé une revue de la littérature médicale afin de déterminer les caractéristiques fondamentales d'un système de rapport d'erreurs. Nous avons ensuite constitué un cahier des charges de notre système déclaratif, le confrontant aux modalités techniques à mettre en place. Ses caractéristiques suivent les recommandations de l'OMS : le système est indépendant, non sanctionnant, rapide, réactif, confidentiel et anonyme tout en permettant un dialogue entre déclarant et analyste, l'analyse est faite par des experts et est basée sur une approche systémique, en ce sens la classification de l'OMS pour la sécurité des patients a été retenue. L'aspect légal a été pris en compte. Puis nous avons conçu l'outil sur le plan logiciel, utilisant des composants "open sources" pérennes, garantissant des évolutions futures. Enfin, l'outil a été testé auprès d'un échantillon d'une dizaine de médecins généralistes. Lors du test, le site a correctement rempli ses objectifs.

[Résumé français] Si globalement l’Internet médical et ses diverses utilisations sont en plein essor et permettent d’améliorer la prise en charge des patients, les sites dédiés aux cabinets de médecine générale commencent quant à eux seulement à émerger en France.Nous avons réalisé un site Internet pour un cabinet libéral de médecine générale selon deux méthodes : la première avec le module Site Express de Wanadoo® qui permet de réaliser un site préformaté, la seconde avec le logiciel Dreamweaver® qui permet de réaliser un site avec le langage HTML.La première méthode est apparue adaptée aux débutants peu formés à l’informatique, étant simple d’utilisation. Elle présente également l’avantage d’être peu coûteuse. Cependant elle est très limitée techniquement.La seconde méthode s’est avérer nécessiter une familiarisation préalable avec le logiciel et s’adresse donc a des utilisateurs plus avertis. Elle est par ailleurs plus coûteuse, mais offre une grande liberté de créativité.En pratique quotidienne, il semblerait que les sites Internet de cabinet de médecine générale favorisent la communication et la relation médecin-patient ; il faut cependant être vigilant au contenu du site qui doit être établi selon les recommandations du Conseil National de l’Ordre des Médecins ainsi que celles de la Commission Nationale de l’Informatique et des Libertés ; il doit être fréquemment actualisé afin d’être en accord avec les recommandations médicales en vigueur, ce qui permet d’augmenter la fiabilité de l’information et de parer au mieux à d’éventuelles attaques juridiques.

This edited collection of articles addresses aspects of medical care in which human error is associated with unanticipated adverse outcomes. For the purposes of this book, human error encompasses mismanagement of medical care due to: * inadequacies or ambiguity in the design of a medical device or institutional setting for the delivery of medical care; * inappropriate responses to antagonistic environmental conditions such as crowding and excessive clutter in institutional settings, extremes in weather, or lack of power and water in a home or field setting; * cognitive errors of omission and commission precipitated by inadequate information and/or situational factors -- stress, fatigue, excessive cognitive workload. The first to address the subject of human error in medicine, this book considers the topic from a problem oriented, systems perspective; that is, human error is considered not as the source of the problem, but as a flag indicating that a problem exists. The focus is on the identification of the factors within the system in which an error occurs that contribute to the problem of human error. As those factors are identified, efforts to alleviate them can be instituted and reduce the likelihood of error in medical care. Human error occurs in all aspects of human activity and can have particularly grave consequences when it occurs in medicine. Nearly everyone at some point in life will be the recipient of medical care and has the possibility of experiencing the consequences of medical error. The consideration of human error in medicine is important because of the number of people that are affected, the problems incurred by such error, and the societal impact of such problems. To be less controlled by others, we learn how emotional systems influence our relationship-oriented brain. The social sciences have been accumulating knowledge since the early fifties as to how we are regulated by others. S. Milgram, S. Ashe, P. Zimbardo and J. Calhoun, detail the vulnerability to being duped and deceived and the difficulty of cooperating when values differ. Murray Bowen, M.D., the first researcher to observe several live-in families, for up to three years, at the National Institute of Mental Health. Describing how family members overly influence one another and distribute stress unevenly, Bowen described both how symptoms and family leaders emerge in highly stressed families. Our brain is not organized to automatically perceive that each family has an emotional system, fine-tuned by evolution and "valuing" its survival as a whole, as much as the survival of any individual. It is easier to see this emotional system function in ants or mice but not in humans. The emotional system is organized to snooker us humans: encouraging us to take sides, run away from others, to pressure others, to get sick, to blame others, and to have great difficulty in seeing our part in problems. It is hard to see that we become anxious, stressed out and even that we are difficult to deal with. But "thinking systems" can open the doors of perception, allowing us to experience the world in a different way. This book offers both coaching ideas and stories from leaders as to strategies to break out from social control by de-triangling, using paradoxes, reversals and other types of interruptions of highly linked emotional processes. Time is needed to think clearly about the automatic nature of the two against one triangle. Time and experience is required as we learn strategies to put two people together and get self outside the control of the system. In addition, it takes time to clarify and define one's principles, to know what "I" will or will not do and to be able to take a stand with others with whom we are very involved. The good news is that systems' thinking is possible for anyone. It is always possible for an individual to understand feelings and to integrate them with their more rational brains. In so doing, an individual increases his or her ability to communicate despite misunderstandings or even rejection from important others. The effort involved in creating your Mindful Compass enables us to perceive the relationship system without experiencing it's threats. The four points on the Mindful Compass are: 1) Action for Self, 2) Resistance to Forward Progress, 3) Knowledge of Social Systems and the 4) The Ability to Stand Alone. Each gives us a view of the process one enters when making an effort to define a self and build an emotional backbone. It is not easy to find our way through the social jungle. The ability to know emotional systems well enough to take a position for self and to become more differentiated is part of the natural way humans cope with pressure. Now people can use available knowledge to build an emotional backbone, by thoughtfully altering their part in the relationship system. No one knows how far one can go by making an effort to be more of a self-defined individual in relationships to others. Through increasing emotional maturity, we can find greater individual freedom at the same time that we increase our ability to cooperate and to be close to others.
