La prévalence des patients diabétiques en hémodialyse est en constante augmentation, en raison de l'augmentation de la prévalence du diabète de type 2 et de l'allongement de l'espérance de vie. Le diabète est un facteur important de morbi-mortalité chez les patients en hémodialyse et leur taux de survie est moins bon que les patients hémodialysés non diabétiques. La prise en charge du diabète de ces patients est particulière puisque les moyens classiques d'évaluation de l'équilibre glycémique tels que l'HbA1c ne sont pas fiables et ne reflètent pas la variabilité glycémique induite par l'hémodialyse. Il a récemment été démontré que le holter glycémique ou CGMS est utilisable pour évaluer l’équilibre glycémique de manière fiable chez ces patients. Nous avons réalisé une étude pilote prospective avant-après, pour évaluer l’impact de l’utilisation des données du CGMS, versus les glycémies capillaires, sur l’équilibre glycémique des patients diabétiques hémodialysés de notre centre. Nos résultats montrent que le holter glycémique ou CGMS est un outil qui permet d'améliorer de manière significative l'équilibre glycémique de ces patients, sans augmenter le risque d'hypoglycémie ni altérer leur qualité de vie. L'utilisation du holter glycémique confirme qu'il existe une variabilité glycémique induite par la dialyse, comme le montrent les profils types de notre étude. Le holter glycémique est donc, comme dans d'autres populations spécifiques telles que les patients diabétiques de type 1 ou les femmes enceintes diabétiques, un outil de choix pour l'adaptation des traitements anti-diabétiques des patients hémodialysés.
