Le [beta]-TCP est un biomatériau synthétique couramment utilisé en chirurgie maxillo-faciale. Il semble induire une faible réponse inflammatoire. Le but de cette étude était d'étudier le degré d'inflammation de la muqueuse de Schneider après une chirurgie de sinus lift en fonction du type de biomatériau greffé. Matériel et Méthodes : 40 sinus (20 patients) ont été inclus et répartis en 2 groupes selon le biomatériau utilisé: biomatériau synthétique (groupe BTCP) ou os naturel (groupe BONE). Un groupe contrôle (groupe CTRL) a été constitué avec les sinus maxillaires non greffés. Douze mesures par sinus ont été réalisées sur les coupes scannographiques pré et post-opératoires permettant d'obtenir une épaisseur moyenne de la muqueuse de Schneider par sinus avant et après la chirurgie. Résultats : la muqueuse de Schneider est significativement plus épaisse dans les groupes BTCP et BONE après la chirurgie. Aucune différence n'a été retrouvée entre les groupes BTCP et BONE sur les mesures post-opératoires. Conclusion : nous avons donc constaté qu'un biomatériau synthétique ([beta]-TCP) n'induit pas plus d'inflammation que l'os naturel. Il constitue donc une alternative intéressante pour les chirurgies de sinus lift.

Sinus floor elevation is often applied to the molar region of the atrophic maxilla to increase in bone mass between the alveolar crestand the sinus floor. Although autogenous bone graft is, so far, gold standard, limitations of donor site bone volume and post-operativemorbidity could be disadvantages. Thus, artificially synthesized bone substitutes, hydroxyapatite (HA) and tricalcium-phosphate(TCP), might be considerably useful materials compared to the autogenous bone. We have developed a novel technique of the sinus floor elevation using plate-shaped beta-TCP in order to perform surgery lessinvasively and to reduce the stress of patients. In this study, we evaluated the prognosis of this technique and the newly developedbiomaterials. Thirteen patients were recruited in this study. The diameter and thickness of plate-shaped beta-TCP were approximately 10 mm and2 mm, respectively. The bony windows were prepared on the lateral wall of the sinus and the sinus membrane was carefully removedand elevated. Then the TCP plate was inserted and supported with dental implants or micro-screws. Finally, bio- resorbablemembrane was placed to cover the bony window and to avoid soft tissue infiltration. The panolamic X-ray and the cone beam CT wastaken every year after the operations to evaluate the volume of newly formed bone, the implant stability, and the remodeling of TCPplates. This clinical research was approved by Ethics Committee, School of Dentistry, Iwate Medical University, and the patient'sconsent was also obtained. Thirteen operations of the sinus floor elevation with plate-shaped beta-TCPs have been performed since 2011. This study consistedof three men and ten women. The average age of all the patients was 53.3 years old. Four of those were staged approach and nine ofthose were simultaneous approach. Ten of 13 patients have had no complications concerning the abnormalities of the maxillary sinusfrom the postoperative to the present, and it passed well. In most cases, newly formed bone in the sinus was observed on X-rayphotos 6 month after the operation. The good stability of the dental implant, at the second stage surgery, was confirmed. TCP platesremained in the same position and achieved the secure space making for long time. Panoramic X-ray and X-ray CT analysesrevealed that TCP-plates were resorbed in 5 years post operation in most cases. There were two patients who complain ofuncomfortable feeling and TCP plates were removed surgically. One TCP plate was spontaneously excreted. In this clinical trial, a new bone formation was observed in the empty space between the TCP plate and the original sinus floor. TCP isa bone-conductive material and might have accelerated bone formation. Applying the newly developed technique with artificiallysynthesized bone substitutes could be a useful method to reduce patientsu2019 stress. This newly established sinus floor elevation maybring better long term prognosis.

Background: Adequate bone volume surrounding dental implants at the time of implant placement has been suggested as a contributing factor forstable peri-implant bone levels. Bone height and density are often limited in the posterior maxilla. Sinus floor elevation is indicated toovercome these anatomic challenges.Aim: The aim of this study was to clinically and radiographically evaluate Osseodensification Crestal Sinus Floor elevation, with and without synthetic and resorbable calcium phosphosilicate putty (Novabone).Materials and Methods: Forty-two implants (Astratech EV) were place in 28 patients using either Osseodensification alone (group 1, n=21) or Osseodensification combined with synthetic and resorbable calcium phosphosilicate putty (group 2, n=21). Insertion torque values were recorded. All implants were allowed to heal submerged, and restored at 4 months. Patients were re-evaluated at 6 and 12 months.Results: No adverse events or complications were observed throughout the study. All implants were successfully restored at 4 months, and remain in function since then. No statistically significant differences were observed in mean insertion torque values ( group 1: 36.4 Ncm; group 2: 39.1 Ncm). Group 2 demonstrated statistically significant superior gains of alveolar ridge height (group 2: 5.9 mm)when compared to group 1 (2.8 mm). All patients were satisfied with both treatment options and reported minimal post-op discomfort.Conclusions: Osseodensification Crestal Sinus Floor elevation is a predictable and safe method for simultaneous placement of dental implants inthe posterior maxilla. Osseodensification promotes adequate insertion torque values in areas of reduced bone height and density for predictable osseointegration of dental implants. Synthetic and Resorbable Calcium Phosphosilicate Putty use in combination with Osseodensification promotes additional vertical augmentation when compared to Osseodensification alone.
