Contexte : En France, de nombreuses mesures ont été mises en oeuvre ces dernières années pour améliorer la prévention des grossesses non désirées et l'accès à l'interruption volontaire de grossesse (IVG) chez les mineures. Méthode : entretiens avec 20 mineures après leur IVG dans 3 centres d'orthogénie afin de recueillir leur parcours contraceptif et médical ainsi que leur ressenti. Résultats : Avant leur IVG, 90 % des mineures ont reçu des informations sur la sexualité et la contraception, cependant leur connaissance reste pauvre : 63 % déclarent peu connaitre leur moyen de contraception et 60 % pensent qu'elles ne peuvent pas être enceintes lors du premier rapport sexuel. Soixante pourcent des mineures utilisaient un moyen de contraception : méthodes naturelles (38 % des cas), préservatif (30 %), pilule (15 %), pilule d'urgence (5 %), mais aucun moyen de longue durée d'action (implant et Dispositif Intra-Utérin (DIU)). La discussion avec les parents était difficile : 30 % des mineures déclaraient parler de contraception avec eux et 15 % de sexualité. De même pour le médecin traitant. La moitié des mineures connaissait le centre d'orthogénie. Toutes les mineures utilisaient un moyen de contraception médicale après leur IVG: pilule isolée (50 %) ou associée au préservatif (20 %), implant (25 %) et le patch (5 %). La différence d'utilisation avant et après l'IVG était statistiquement significative pour la pilule et l'implant. Aucune pilule d'urgence ou DIU ont été prescrits. Cinq pourcent des mineures veulent reparler de leur IVG avec un professionnel de santé. Conclusion : Des efforts sont encore à faire dans la prise en charge de ces mineures en prévention primaire et secondaire. Les parents, les adultes référents, les professionnels de santé et de l'éducation nationale doivent se sentir concernés. Les contraceptions de longue durée d'action peuvent être davantage proposées.
