Contexte : la visite médicale constitue un dispositif majeur de la promotion pharmaceutique. Son influence sur les médecins est connue. Il existe un déficit d'évaluation des enseignements concernant la promotion pharmaceutique faite par les visiteurs médicaux pour conclure à un impact concret des enseignements sur les étudiants. Objectif : élaborer et valider un questionnaire d'évaluation des connaissances et des compétences des étudiants pour appréhender l'information fournie par le visiteur médical. Matériel et méthode : élaboration d'un questionnaire à partir de référentiels. Validation des caractères métrologiques du questionnaire dans le cadre d'une première expérience d'enseignement auprès de 19 étudiants aux enjeux de la promotion pharmaceutique, intégrée au DES de médecine générale à l'Université de Bordeaux. Comparaison des scores avant et après la formation et comparaison entre les 19 participants à la formation et les 19 témoins. Résultats : bonnes propriétés psychométriques pour la plupart des critères de validation : Bon taux de réponse (90.12%). Excellente fidélité inter-juge (coefficients de Kappa autour de 0.8). Bonne consistance interne (coefficient alpha de Cronbach à 0.63). Structure dimensionnelle de l'instrument superposable aux 2 facettes déterminées lors de l'élaboration du questionnaire : connaissances et compétences. Corrélation correcte (Coefficient de corrélation 0.53) entre le score au questionnaire et la perception de l'influence du visiteur médical. Amélioration significative du score global des participants au questionnaire par la formation. Conclusion : le questionnaire élaboré et validé pourrait contribuer à une évaluation fiable et validée des améliorations des connaissances et compétences pour appréhender le discours du visiteur médical des étudiants ayant suivi l'enseignement et à l'amélioration du contenu de l'enseignement.

La visite médicale a pour rôle de favoriser le bon usage des médicaments grâce à la qualité de 1'information délivrée aux médecins par les entreprises qui les créent, les produisent et les distribuent. La visite médicale est un axe de communication majeur pour les laboratoires pharmaceutiques afin d'assurer la promotion de leurs produits auprès des professionnels de santé. Différentes mesures ont été prises au cours du temps pour encadrer les pratiques et une Charte de la visite médicale a été instaurée. La promotion répondant à un enjeu économique important, des pratiques contraires à la transmission d'information sur le bon usage du médicament sont constatées. Les scandales sanitaires récents ont révélé des manquements à la réglementation. Ils ont été à l'origine d'une réforme en profondeur de la visite médicale et notamment d'un encadrement plus strict. Les dernières mesures prises lors de la réforme de la sécurité sanitaire et du système de santé et la publication de la loi Bertrand instaurent en particulier un contrôle de la publicité a priori. La loi prévoit également une visite collective à l'hôpital. Elle modifie les dispositions de la loi DMOS et impose la transparence des liens en rendant publiques les conventions établies entre les laboratoires et les professionnels de santé. L'application de la loi est cependant longue et difficile, des décrets sont encore à paraître. Cet encadrement plus strict pousse les laboratoires à revoir leurs pratiques, voire à restreindre la visite médicale et à se tourner vers d'autres axes de communication. Une redéfinition des fondamentaux de la visite médicale, de la déontologie professionnelle et de la Charte de la visite médicale a été nécessaire afin de garantir une information claire, loyale et exhaustive. Une chose est certaine : 1' exercice de la profession de visiteur médical évoluera encore durant les prochaines années
