Introduction : la médecine générale rurale représente un exercice professionnel difficile et peu attractif. L'objectif principal est d'évaluer les difficultés rencontrées par les médecins généralistes installés en zone rurale en Corse dans le cadre de leur pratique. Matériel et méthode: cette étude qualitative a été menée auprès de médecins généralistes installés en Corse. Le recueil de donnée s'est déroulé entre le 31 Août et le 7 Décembre 2017 où ont été effectués des entretiens semi-dirigés : six en face à face et huit par entretien téléphonique. Résultats : 14 entretiens ont été réalisés et ont mis en évidence trois grands types de difficultés. Les premières sont liées à l'offre de soin avec une densité médicale qui se dégrade ; des manques dans l'accès à certaines spécialités ou services hospitaliers ; une déficience en capacité d'accueil des personnes âgées et atteinte de handicap ; l'absence de CHU. Viennent ensuite les difficultés liées à la pratique de la médecine générale : éloignement des structures de soin ; surcharge administrative et de travail majorée l'été ; gestion des urgences parfois complexe; formation continue déficiente sur le territoire avec réticence aux nouveaux moyens de formation; difficultés à se faire remplacer ; absence d'anonymat. Enfin, les difficultés d'ordre structurel en rapport avec un réseau routier dangereux ; un réseau numérique, internet et téléphonique peu fiable. Conclusion : les structures corses doivent évoluer pour permettre, par l'apport des nouvelles générations, de développer une médecine en phase avec son temps, pleinement inscrite dans le 21ème siècle.
